Job Opportunity: Pelvic Health Physical Therapist (Outpatient)

Location: Jacksonville, Illinois
Employment Type: Travel/Contract
Contract Length: 13 weeks

Position Details

• Specialty: Physical Therapy - Pelvic Health
• Patient Population: Male and female pelvic floor patients
• Shift: Monday to Friday, 8:30 AM to 5:00 PM
• Experience Required: Minimum 1 year
• First-time Travelers: Accepted
• Weekend Requirement: None
• On-call: No
• Floating: No
• Block Schedule: Not available, fixed M-F schedule
• Local Candidates: Accepted, no radius restrictions
• Licensure: Must have an active Illinois PT license or have already applied
• Return to Office (RTO) Restrictions: Ideally no more than 5 days
• Charting System: Cerner (experience preferred)
• Dress Code: Business casual or plain solid scrubs
• Parking: Free in any unrestricted space

Required Certifications & Licenses

• Illinois State Physical Therapist License (active or applied)
• Basic Life Support (BLS) certification
• ACLS, PALS may be required based on unit/specialty
• Primary source verification of licenses and certifications

Compliance & Background Requirements

• Background check within 30 days of start, covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM, HHS/OIG checks within 30 days prior to start
• Fingerprinting must be completed in Illinois 3 business days prior to start (Illinois Health Care Worker Registry Search replaces fingerprinting effective 7/8/2025)
• Education verification (degree/transcript/diploma)
• State-specific searches including Illinois Department of Corrections, Most Wanted, and State Police Murderer and Violent Offender Against Youth Registry within 30 days of start
• Competency exam required for all clinical candidates
• References: 2 professional references required for clinical candidates

Health & Safety Requirements

• Physical exam within 1 year of start, signed by an MD, NP, PA, or equivalent
• Fit mask test required (specific mask types accepted)
• Negative TB test or chest X-ray within 1 year
• TB questionnaire updated annually
• Immunizations or proof of immunity (or signed declination) for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 30 days of start
• Seasonal flu vaccination or approved medical/religious exemption required annually by November 30
