Job Opportunity: Travel RN - Medical/Surgical

Location: Cleveland, Ohio
Shift: Day shift, 36 hours per week, every other week (EOW) and every other holiday (EOH)
Contract Length: 12 weeks


Position Summary

Seeking a Registered Nurse (RN) with Medical/Surgical specialty for a travel assignment.


Required Certifications & Licenses

  • BLS (American Heart Association) – Required for clinical positions
  • Valid Permanent RN License – Primary source verification required within 30 days of start; temporary licenses not accepted
  • License/Certificate Verification – Must be current and verified
  • Ohio Fingerprints – Background check including BCI&I and FBI fingerprints depending on residency duration in Ohio
  • UHS I-9 Attestation – Proof of eligibility to work
  • Driver's License/State ID – Non-expired, required for submission

Immunizations & Health Requirements

  • COVID-19 Vaccine – Preferred but not required; medical or religious exemptions accepted
  • MMR (Measles, Mumps, Rubella) – Positive titers within last 15 years required; booster protocol if titers low
  • Varicella – Positive titers within last 15 years required; booster protocol if titers low
  • Hepatitis B – Proof of full series, partial series with declination, or positive titers within 15 years
  • Tdap – Vaccination within last 10 years; no declinations accepted
  • Influenza Vaccine – Required during flu season (Oct-Mar); declination requires mask-wearing and specific forms
  • 2-Step PPD or IGRA (Quantiferon/Tspot) – Negative test within last 6 months; if positive, clear chest x-ray and TB questionnaire required
  • Positive PPD – Clear chest x-ray and annual TB questionnaire required
  • Fit Test – Completed within 1 year of start; specific approved respirators listed
  • Physical/Health Statement – Pre-employment physical exam within 1 year stating ability to work without restrictions
  • Drug Screen – 10-panel drug screen within 60 days of start; instant drug screens not accepted

Background & Verification Requirements

  • Criminal Background Check – Within 6 months, including Social Security Trace, county searches, national sex offender registry
  • Social Security Trace – If not included in background check, within 1 year of start
  • National Database Criminal Background Check – For Ohio residents, FCRA-compliant
  • OIG Search – Within 6 months prior to start; no attestations allowed
  • SAM/EPLS/GSA/OFAC Screening – Within 6 months prior to start; no attestations allowed
  • Employment Verification – Verification of employment within last 5 years through third-party; no attestations allowed
  • Education Verification – Highest level of education verified through third-party; international equivalency required if applicable
  • UHS Ohio Fingerprints – Required as per Ohio residency status
  • UHS NSO (National Service Office) – If not run with background check, within 30 days of start

Competency & Training Requirements

  • UHS Competency Exam – Must score at least 80%, completed within 1 year prior to start
  • UHS Core Competencies – Includes HIPAA, Joint Commission-NPSG, OSHA topics; 80% passing score required; completed within 1 year prior to start
  • UHS Skills Checklist – Clinical skill-specific checklist completed within 1 year prior to start (initial hire only)
  • UHS Module Completion Certificate – Completion of required safety and compliance training modules prior to clearance; annual renewal required
  • UHS Submission Packet – Resume, 2 professional references (within last 2 years), job application, and employment history form required for submission
  • UHS Badge Photo – Recent headshot (shoulders up, plain background) in JPEG format

Additional Notes

  • Temporary licenses are not accepted; must have a valid permanent RN license.
  • Attestations are generally not accepted for verifications and must be supported by official documentation.
  • Some immunization boosters require follow-up titers and documentation of certification dates.
  • Masking and additional forms required if influenza vaccine is declined.
  • Background checks and drug screens have specific timing and validity requirements.

This position is ideal for RNs with Medical/Surgical experience who meet the above credentialing and compliance requirements and are interested in a 12-week travel assignment in Cleveland, Ohio.
