Job Opportunity: RN Medical / Surgical / Telemetry

Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks (approximately 3 months)
Schedule: Every Other Week (EOW)

Required Credentials & Certifications

Licenses & Certifications (Must be current and verified within 30 days of start)

• Registered Nurse License (state verification required, upload scanned online verification)
• BLS (Basic Life Support)

  • American Heart Association certification required for all direct patient care personnel
  • Upload front and back images of card with expiration date
• ACLS (Advanced Cardiovascular Life Support)

  • Required for RNs in CCU, ED, PACU, MICU, SICU
  • Upload front and back images of card with expiration date
• NIHSS (National Institutes of Health Stroke Scale)

  • Required
• TNCC (Trauma Nursing Core Course) (optional)

Professional Requirements

• Signed Trinity Code of Conduct Form
• Completed Trinity I9 form
• Signed Confidentiality of Patient and Proprietary Information and Information System Security Agreement
• Signed Trinity Integrity and Audit Services Form (signed shortly before start)
• Completed Trinity Non Employee Upload Form (pages 1-3 and 28)
• Completed Trinity Conflict of Interest Disclosure Form
• Federal, Nationwide, and County Criminal Background Check (last 7 years, including SSN verification) dated within 30 days of start
• Education Verification (highest academic degree or diploma verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• OIG (Office of Inspector General) exclusion check dated within 30 days
• Joint Commission CORE Exam within the last year
• NSO (National Sex Offender) check dated within 30 days (includes all aliases and locations)
• Completed System Access Form one week before start
• Child and Adult Dependent Abuse Training (Iowa only)
• Iowa-specific Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days

Health & Immunizations

• Fit Test
• Trinity 10 Panel Drug Screen (completed within 30 days of start, includes 10 specific substances; no THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption with agency letterhead; declination not accepted in CA or IL)
• MMR (proof of 2 vaccinations or IgG titer lab report)
• Hepatitis B (must have started vaccination series prior to start; titers not accepted)
• Influenza Vaccine (proof of vaccine, declination, or exemption; declination not accepted in CA or IL; valid for flu season 10/1/2024–4/30/2025)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) (positive TB requires chest x-ray within 1 year for some states, 5 years for others)
• Varicella (proof of 1 vaccination or IgG titer lab report)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Tdap (current, non-expired vaccine; declinations accepted if no pediatric interaction)
• Trinity Color Vision test

Competency & Skills

• Trinity Specialty Specific Competency Testing (specialty exam completed within last year)
• Trinity Skills Checklist (provided by agency for clinical contractors)

Submission Requirements
