Job Description

• Provide home health physical therapy services including home visits and periodic office meetings.
• Average patients per day: 5-6
• Patient visits include Start of Care and Follow Ups.
• Patient load: 5-10 patients per day, equating to roughly 6-8 points daily (30 points per week expected).
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (standard 5x8 schedule). Some positions may have alternate schedules (e.g., 4x10 shifts).
• Work locations: Patient homes and Assisted Living Facilities (ALF).
• Must be willing to travel up to 75 miles from the home clinic; typical travel 50-60 miles per day.
• No guaranteed hours; if patient census is low, floating to nearby clinics may be arranged.
• On-call requirements vary by location; discuss during interview.
• Weekend requirements vary; discuss during interview.

Requirements

• Minimum 1 full year of recent adult home health experience required.
• First-time home health workers not accepted.
• Current, valid license required (pending licenses accepted only if issuance is imminent).
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Reliable transportation and current driver's license required.
• Agency-required liability insurance required.
• Must be able to use EMR systems including HomeCare HomeBase, Oasis, and Outlook.

Productivity Expectations

• Weekly productivity: 25-30 units or approximately 32 points per week.

Additional Information

• Mileage reimbursed at IRS standard rate; compensation is from patient home to patient home (commute to/from first and last patient is not billable).
• Locals accepted at the same rate.
• This is a contract assignment with potential for hire at the end of the contract.
• Coverage area: Monroe County, Georgia.

Compliance and Documentation Requirements

Candidates must complete and maintain the following prior to start and during assignment as applicable:

Licensure & Certifications

• Valid PTA license for Georgia.
• CPR/BLS certification.

Background Checks & Authorizations

• Standard background check covering last 7 years of residence/employment/school.
• Sex offender search.
• EPLS/GSA/SAM and HHS/OIG checks.
• State-specific background checks as required.
• Fingerprinting as required by state regulations.

Health & Immunizations

• Physical exam within 12 months confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months.
• Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
• Drug screening (10-panel).
• Seasonal flu and COVID-19 vaccination documentation or declination.

Facility-Specific Documents & Training

• Personal or auto insurance policy (must be current and under candidate’s name or list candidate as driver).
