Description

Job Opportunity: Ultrasound Technician

Location: Mason City, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Work Schedule

  • Rotating 8 and 10 hour shifts, Monday through Friday
  • Earliest start time: 6:30 AM
  • Latest end time: 5:00 PM

Preferred Experience & Skills

  • Experience with Epic & GE Centricity PACS
  • BLS (Basic Life Support) certification
  • RDMS (Registered Diagnostic Medical Sonographer) or RVT (Registered Vascular Technologist) credentials preferred

Required Credentials & Onboarding Documentation

Certifications & Licenses

  • BLS (American Heart Association certification required for all direct patient care personnel)
  • Current license/certification verification dated within 30 days of start

Health & Immunizations

  • Fit Test
  • Trinity 10 Panel Drug Screen (completed within 30 days of start, no THC)
  • COVID-19 Vaccine (proof of booster by Sept 2025, or approved declination/exemption)
  • MMR (proof of 2 vaccinations or IgG titer)
  • Hepatitis B vaccination series (must have started prior to start date)
  • Influenza Vaccine (proof or declination for 2024-2025 season)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
  • Varicella (proof of vaccination or IgG titer)
  • Physical/Health Statement (exam within last year confirming fitness to work)
  • Tdap vaccine (current, non-expired; declination accepted if no pediatric interaction)

Professional & Compliance Forms

  • Trinity Code of Conduct Form (signed)
  • Trinity I9 form
  • Confidentiality and Information System Security Agreement (signed)
  • Integrity and Audit Services Form (signed shortly before start)
  • Non Employee Upload Form (pages 1-3 and 28 completed)
  • Specialty Specific Competency Testing (copy of specialty exam within last year)
  • Conflict of Interest Disclosure Form (signed)
  • Criminal Background Check (federal, nationwide, county for last 7 years, SSN verification, dated within 30 days)
  • Education Verification (highest academic degree or diploma verified)
  • EPLS (Excluded Parties List System) report dated within 30 days
  • GSA/OIG Exclusion report dated within 30 days
  • Joint Commission CORE Exam within last year
  • NSO (National Sex Offender) check dated within 30 days
  • System Access Form (completed and uploaded one week before start)
  • Skills Checklist (provided by agency for clinical contractors)
  • Child and Adult Dependent Abuse Training (Iowa only)
  • Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start)
  • OFAC (Office of Foreign Assets Control) sanctions check within 30 days

This position requires thorough credentialing and compliance with health and safety standards prior to start. Candidates should be prepared to provide all required documentation and certifications as outlined.
