Position Details

• Role: PTA Outpatient
• Shift Options:

  • 5 days per week, 8-hour shifts (8:30 AM - 5:00 PM)
  • Or 4 days per week, 10-hour shifts
• Experience Required: Minimum 1 year
• First-time Travelers: Accepted
• Weekend Requirement: No weekends required
• On-call: No
• Floating: May float within scope to meet facility needs
• Schedule: Monday through Friday, block schedule
• Locals Accepted: Yes, no radius restrictions
• License: Must have active Illinois PTA license or be in application process with ETA to apply
• Return to Office (RTO) Restrictions: Ideally no more than 5 days

Compliance & Onboarding Requirements

• Valid US Driver’s License or State ID
• Background Checks covering all locations lived/worked in past 7 years (including SSN trace, national criminal, sex offender, FACIS Level 3)
• EPLS/GSA/SAM, HHS/OIG checks within 30 days prior to start
• Illinois-specific searches (Department of Corrections, Most Wanted, State Police Murderer and Violent Offender Against Youth Registry) within 30 days prior to start
• Illinois Health Care Worker Registry Search (fingerprints must be taken in Illinois 3 business days prior to start)
• Education Verification (degree verification required)
• Competency Exam (required for all clinical/allied candidates)
• Photo for badge (color photo, mid-shoulders up, plain background)
• Physical Exam within 1 year of start (must indicate good physical and mental health, free of communicable disease, able to perform job functions)
• Fit Mask test (required for new contracts)
• Negative TB test or Chest X-Ray within 1 year
• TB Questionnaire (updated annually)
• Immunizations or documentation for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel Drug Screen within 30 days of start
• Flu Vaccination or approved Medical/Religious Exemption required annually by 11/30
• References: 2 professional references required for clinical candidates
