Position Details

• Specialty: Physical Therapy - Outpatient Rehab
• Shift: 4 days a week, 10-hour shifts (06:45 AM - 5:00 PM)

  • Flexible shift options available (8 or 10 hours)
• Weekends: No weekends required
• Experience Required: Minimum 1 year
• First-time Travelers: Accepted
• Certifications Required: BLS (Basic Life Support)
• Schedule: Open to block scheduling (discuss with manager)

Required Credentials & Documentation

Certifications

• BLS (Basic Life Support) - AHA or RQI acceptable
• ACLS, PALS may be required based on unit/specialty
• State License verification (Primary Source Verification)
• Tech Certification Verification (Primary Source Verification)
• Orientation Safety Manual Acknowledgement

Background Checks

• Comprehensive background check within 30 days of start (covering all locations lived/worked in past 7 years)
• EPLS/GSA/SAM check within 30 days of start
• HHS/OIG check within 30 days of start
• Illinois-specific searches including Department of Corrections, State Police Murderer and Violent Offender Against Youth Registry, Most Wanted, and Healthcare Worker Registry (fingerprints must be done in Illinois 3 business days prior to start)

Health & Immunization Requirements

• Physical exam within 1 year of start, signed by an MD, NP, PA, or DO
• Fit mask test (3M 1860, 1870, or 1870+) required for new contracts
• Negative TB test or chest X-ray within 1 year
• TB questionnaire updated annually
• Immunizations or proof of immunity for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 30 days of start
• Seasonal flu vaccination or approved medical/religious exemption required annually by November 30

Facility Specific Documents

• Badge photo (color, mid-shoulders up, plain background) due before start
• Code of Conduct signed within 7 days of contract signing
• Parking permit form signed within 7 days of contract signing (if applicable)
• Badge name confirmation

Competencies & History

• Competency exam required for all clinical/allied candidates
• Skills checklist required for clinical/allied candidates
• Two professional references required for direct travel, allied, per diem, and acute care candidates

Additional Notes

• Fingerprinting for Illinois Healthcare Worker Registry is mandatory for clinical candidates without an Illinois state license providing direct care. Fingerprints must be completed in Illinois and cannot be done out of state.
