Registered Nurse Travel/Contract Position

Location: Berlin, Vermont
Contract Length: 16 weeks

Job Description

• Acute Med-Surg Telemetry RN role
• Responsibilities include:

  • NG tube insertion
  • Peripheral IV insertion
  • Telemetry strip interpretation at bedside
• Schedule: Minimum every other weekend or adjusted to unit needs
• Must work 2 out of 4 major holidays (Thanksgiving, Christmas, Christmas Eve, New Year's Day)
• No block schedules; work Monday through Sunday
• Bi-weekly orientations on Mondays

Requirements

• Certifications:

  • AHA BLS (Basic Life Support)
  • AHA ACLS (Advanced Cardiac Life Support)
  • NIHSS (National Institutes of Health Stroke Scale) required before starting
  • Telemetry certification preferred
• Experience:

  • 5 years Med-Surg Telemetry experience OR 3 years Telemetry experience
  • Prior travel experience required
  • Completion of 2 travel assignments in 300+ bed facilities OR previous experience at any UVMHN acute care facility
• EPIC EMR experience within the past 2 years is mandatory
• Reliable personal or rental vehicle strongly recommended (no public transportation available)
• Vehicles should have all-season or snow tires from November through April

Orientation & Scheduling

• Orientation offered bi-weekly on Mondays
• No block scheduling allowed
• Must explain any employment gaps over 90 days
• Complete and detailed work history required (bed count, EMR, facility type, etc.)

Background Checks & Verifications

• Background check within 30 days of start covering all locations lived/worked/schooled in past 7 years
• Education verification based on job requirements
• State license verification and NURSYS verification within 30 days of start and updated annually
• Vermont Adult and Child Abuse Registry consent form required
• Vermont Crime Information Center (VCIC) criminal background check required for Vermont placements
• COVID-19 vaccination attestation and questionnaire required
• Drug testing required for NY facilities (not for VT)

Health & Safety Requirements

• Physical exam within 12 months of start
• N95 Fit Mask test required prior to assignment and updated annually (approved mask types specified)
• Tdap vaccination within past 10 years
• Negative TB test or chest X-ray within past 12 months
• MMR (Measles, Mumps, Rubella) vaccination or positive titer required
• Hepatitis B series and titer or signed declination
• Varicella vaccination or positive titer
• Seasonal flu vaccination or approved exemption required (strict for VT placement)
• Pneumonia vaccination or declination required for NY facilities

Competencies & Exams

• Medication/Pharmacology competency exam required
• Color blind testing required
• Other competency exams as required by specialty

Facility Specific Requirements

• Passport-style photo for badge (plain background, no logos or hats)
• Review dress code and telephone usage policies prior to start
• Complete auto registration form within 48 hours of signed contract
• Complete EPIC and Cornerstone training modules prior to start (approx. 8 hours, non-billable)
• COVID testing protocol on Day 1 of work (if required)
• Emergency contact form completion required
• Hepatitis B and Pneumonia vaccination declination forms if applicable
• NameTag verification setup required
• Workday information attestation required

References & Documentation

• Two professional references from assignments within last 3 years
• Employment application required
• Skills checklist within 12 months
• Verification of 2 years of most recent clinical work history

Additional Notes

• Public transportation is not accessible; reliable transportation is essential
• Vehicle must be equipped for winter conditions during November-April
• Must comply with all facility-specific policies and documentation deadlines
