Requirements & Compliance

Background Checks

  • 7-year background check required within 30 days of start; must be updated annually.
  • Must include Social Security trace, county searches for last 7 years, nationwide sex offender check.
  • EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start.

Certifications (as applicable)

  • ACLS (AHA only: Instructor, Provider, RQI accepted)
  • BLS (AHA only: Instructor, Provider, RQI accepted)
  • PALS (AHA only: Instructor, Provider, RQI accepted)
  • NRP (Neonatal Resuscitation Procedure)
  • CHEMO (Chemotherapy)
  • TNCC (Trauma Nursing Core Course)
  • EFM (Electronic Fetal Monitoring)
  • ENPC
  • NIH Stroke Certification
  • Pass/Fail Testing (EKG/Medication) required for RNs
  • Tech Certification Verification (National) within 30 days of start
  • Orientation Safety Manual Acknowledgement (annual update)
  • State License Verification within 30 days of start
  • NURSYS Verification for RN, LPN/LVN, AP within 30 days of start
  • Acknowledgement of Pass/Fail Testing (for RNs)

Competency Exams

  • Medication Test/Pharmacology Competency Exam as applicable by profession.
  • Must meet or exceed Angoff score or pass if not done through Relias.

Facility-Specific Documents (Medical Center Hospital)

  • Notice of Employment (update if >30 day gap; add "traveler" under title)
  • Standards of Conduct (update if >30 day gap; add "traveler" under position)
  • Dress Code Policy Acknowledgement (update if >30 day gap)
  • Agency Checklist (to be completed by clinician or agency; add "traveler" under title)
  • Standards of Behavior (update if >30 day gap)
  • COVID Declination (if declining COVID vaccine; re-complete if >30 day gap)
  • HIPAA and Fair Warning Quiz Modules (complete within 72 hours of signing)
  • Orientation and Safety Training Modules (complete within 72 hours of signing)
  • Annual TB Questionnaire (update annually or if >30 day gap)

Health Requirements

  • Physical Exam within 1 year of start; must be completed within 72 hours of contract signing.
  • Fit Mask Test within 1 year; update annually. Acceptable masks: Kimberly Clark Technol N-95 or Halyard Orange Duckbill. PAPR not accepted.
  • Tetanus/Diphtheria/Tdap vaccine within 10 years of start.
  • Negative TB test or Chest X-Ray (PPD, QGold, or TSpot within 1 year; chest X-ray within 2 years for positive responders); annual TB questionnaire required.
  • Rubeola (Measles): Positive titer or 2 vaccines required; booster if low/equivocal titer.
  • Mumps: Positive titer or 2 vaccines required; booster if low/equivocal titer.
  • Rubella: Positive titer or 2 vaccines required; booster if low/equivocal titer.
  • Hepatitis B: Positive titer, series (3), or declination required.
  • Varicella: Positive titer or 2 vaccines required; booster if low/equivocal titer.
  • Drug Testing: Negative 10-panel drug screen within 30 days of start; must have plan to complete within 72 hours of contract signing.
  • Flu Seasonal Vaccination or Declination required during flu season (Oct-April); mask required if declined until March 31.
  • COVID-19 Vaccine preferred but not required; collect if candidate has.

History & Documentation

  • Two references required within 3 years of start.
  • Copy of resume or employment application required.
  • Skills checklist required; update annually.
