Job Opportunity: OR Technician (Travel)

Location: Ravenna, Ohio
Contract Length: 12 weeks

Work Schedule

• Monday to Friday, 8-hour day shifts
• On-call 1-2 times per week and every 4th weekend
• Response time for call: 30 minutes

Required Certifications & Credentials

• BLS (American Heart Association) – Required for clinical positions
• License/Certificate Verification – Permanent valid license required (no temporary licenses)
• MMR – Positive titers within last 15 years; booster protocol if low titer
• Fit Test – Completed within 1 year of start, updated annually
• Hepatitis B – Proof of full series, partial series with declination, or positive titers within 15 years
• Varicella – Positive titers within last 15 years; booster protocol if low titer
• Tdap – Vaccination within last 10 years, no declinations accepted
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – Negative within last 6 months; annual requirement
• Influenza Vaccine – Required October through March; declination requires mask and specific forms
• Physical/Health Statement – Pre-employment physical exam within 1 year, must state ability to work without restrictions
• Drug Screen – 10-panel drug screen within 60 days of start; no instant screens allowed

Background & Verification Requirements

• Criminal Background Check – Within 6 months, includes Social Security Trace, County searches, National Sex Offender check
• Ohio Fingerprints – Required for Ohio residents (5+ years or less than 5 years with FBI check)
• Education Verification – Highest level verified through third-party; international equivalency required if applicable
• Employment Verification – Verification of employment within last 5 years through third-party
• OIG Search – To be completed within 6 months prior to start
• SAM/EPLS/GSA/OFAC Screening – Completed within 6 months prior to start
• Social Security Trace – If not included in background check, to be completed within 1 year of start
• National Database Criminal Background Check – For Ohio residents to meet fingerprint requirements

Competency & Training Requirements

• UHS Competency Exam – Must score at least 80%, completed within 1 year prior to start
• UHS Core Competencies – Includes HIPAA, Joint Commission, OSHA topics; 80% passing score required
• UHS Skills Checklist – Clinical skill-specific checklist, completed within 1 year prior to start
• UHS Module Completion Certificate – Safety and compliance training modules, completed prior to clearance and renewed annually

Additional Requirements

• UHS I-9 Attestation – Proof of eligibility to work in the U.S.
• UHS Submission Packet – Resume, professional references (within last 2 years), job application, and employment history form
• UHS Badge Photo – Recent headshot (shoulders up, plain background, JPEG)
• Driver's License/State ID – Non-expired photo ID required

• Temporary licenses are not accepted; permanent valid licenses only
• Attestations are generally not accepted unless specifically noted
• Some vaccinations and titers require boosters and follow-up testing as per protocol
• Masking and specific forms required if influenza vaccine is declined
