Job Opportunity: Physical Therapist (PT) - Travel/Contract

Location: Plainview, Texas
Specialty: Therapy/Rehabilitation

Contract Details

  • Length: 13 weeks
  • Floating: No floating between units
  • On-call: Not required
  • Weekends: Not required
  • Block Schedules: Yes
  • Patient Ratios: 1:1

Certifications Required

  • ACLS (Advanced Cardiac Life Support): AHA or Canadian Heart & Stroke Foundation certification
  • BLS (Basic Life Support): AHA or Canadian Heart & Stroke Foundation certification
  • PALS (Pediatric Advanced Life Support): AHA or Canadian Heart & Stroke Foundation certification
  • NRP (Neonatal Resuscitation Procedure): Current certification by AAP, AHA, or Canadian Paediatric Society
  • Chemotherapy Certification: Verify if required per clinical track
  • Electronic Fetal Monitoring (EFM): If applicable, AWHONN or NCC-EFM certification
  • IV Certification: As applicable (LVN/LPN/Other)
  • Management of Assaultive Behavior (MAB): If required
  • NIH Stroke Certification: Required every 2 years if applicable
  • CPI (Crisis Prevention Institute): If required
  • State License: Must have valid professional license
  • NURSYS Verification: Required for RNs
  • Tech Cert Verification: For allied professionals as applicable

Background and Compliance Requirements

  • Background check within 45 days of start, covering all locations lived/worked in past 7 years
  • E-Verify attestation required
  • Education verification required for all candidates
  • State-specific searches (e.g., Washington WSP/WATCH) as applicable
  • Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
  • References: Minimum 2 supervisor references covering 1 year employment in last 3 years (6 months for CNAs)
  • Employment application covering at least last 5 years (7 years for APN/Subcontractors)
  • Skills checklist updated annually as applicable

Health and Safety Requirements

  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO
  • Fit mask test (N95 or equivalent), updated annually (travel only)
  • Immunizations/vaccinations:
    • Tetanus/Diphtheria/Tdap (within 10 years or declination)
    • Negative TB test or chest X-ray within 12 months, updated annually
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer
    • Seasonal flu vaccine or declination (required during flu season)
    • COVID-19 vaccine or declination (required if onsite)
  • Respiratory safety clearance for respirator use (travel only)

Facility and Job Specifics

  • Wear street clothes in OR
  • Free parking in unrestricted spaces
  • Block scheduling required
  • No weekend or on-call requirements

Training and Competency

  • Medication test/pharmacology competency exam as applicable
  • Competency exams required by specialty
  • Color blind testing if required by unit
  • WorkRamp LMS modules required pre-start and annually, including:
    • Core modules by profession and region
    • Specialty-specific modules for ER, L&D, leadership roles
    • Regional core and specialty modules as applicable
  • Epic training modules required for certain regions and roles

Additional Notes

  • Float policy acknowledgement required (may be required to float to nearby Providence facilities)
  • Passport-style badge photo required
  • Some state-specific and facility-specific documents and attestations may be required depending on placement location

This position is suitable for first-time travelers and offers a structured schedule with no weekend or on-call duties, ideal for therapists seeking a focused acute care experience in a Level IV trauma center setting.
