Job Description
- Skilled home care experience required (private duty nursing experience only not accepted)
- Must have home health experience with OASIS training and experience
- Required skills:
- IV skills and wound care
- General medical diagnoses
- High-risk diagnoses including Heart Failure (HF), COPD, Myocardial Infarction (MI), Pneumonia, Stroke
- Post-operative care
- Certifications required: BCLS
- Scrubs: Any solid color scrub
Unit Information
- Patient care includes cardiopulmonary assessment, basic wound care, ABI compression dressing, wound vac dressing, Pleurx/Aseptdrain care, foley catheter change and care, staple/suture removal, glucometer use, pro-time machine use, SQ/IM injections, lab draw venipuncture, IV skills, PICC line or Midline care and dressing change, SQ port access/deaccess
- Skilled Home Care visits (Medicare, Medicare Advantage, Commercial insurance)
- 6-8 home skilled visits per day
- Shift hours vary based on HomeCare office needs:
- Monday - Friday 8am to 5pm or 9am to 6pm
- Thursday - Monday or Friday - Tuesday shifts
- Alternating weekend shifts
- Visits and hours are self-scheduled based on office needs
Requirements
- Minimum 1 year of experience in home health nursing
- Valid driver’s license and reliable car for home visits
- COVID-19 vaccination or approved exemption on file
- Flu shot or declination required; mask wearing mandatory at all times while at work
- Laptop, MiFi for internet access, and portable printers provided
- Documentation systems used: BayCare Forms, BayCare Scheduling, Ace Timesheet
Certifications & Competencies
- BLS (Basic Life Support) - AHA or RQI accepted
- ACLS (Advanced Cardiac Life Support) - AHA or RQI accepted (if applicable)
- Other certifications as applicable (PALS, NRP, EFM, CPI, etc.) depending on unit requirements
- Medication Test/Pharmacology Competency Exam required (score 80% or higher)
- Dysrhythmia/EKG Competency Exam required for certain units (score 100% on lethal arrhythmia section)
- Stroke Education CEUs (8 total credits required for relevant units)
- State license primary source verification required
- NURSYS verification required for RNs and LVNs
- Color Blind Testing (Relias) required prior to start
Health & Safety Requirements
- Physical exam within 6 months of start
- Negative TB test or chest X-ray within 10 weeks of start (or chest X-ray and TB questionnaire if prior positive)
- Annual TB questionnaire required in lieu of annual TB test
- Required immunizations or titers: Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella (with boosters if titers low/equivocal)
- Standard 9-panel drug test within 30 days of start (instant tests not accepted)
- Seasonal flu vaccination or declination with mask requirement
- COVID-19 vaccination or approved exemption required
- Respirator medical clearance annually if respirator use is required
Documentation & Compliance
- Driver’s license must remain active and current throughout assignment
- Background checks required within 1 year of start, including SS trace, county searches, nationwide, sex offender, education verification
- Foreign equivalency evaluation required if education completed outside USA or Puerto Rico
- Fingerprinting: Florida AHCA Level II fingerprint clearance required prior to start (Livescan electronic fingerprinting)
- Photo for badge required prior to start (clear, professional, solid background)
- Personal or auto insurance required for home health positions (must be current throughout assignment)
- BayCare Health MSP dress code and tobacco policy must be followed
- BayCare online learning modules must be completed prior to start (4-8 hours, deadline Thursday before start)
- BayCare Health MSP fit mask acknowledgement required
- BayCare Health MSP EMR signature request form required for home health candidates
- References: 2 references from past 2 years required
- Employment application and skills checklist required
Additional Notes
- Candidates must be able to self-schedule visits and hours based on HomeCare office needs
- Laptop, MiFi, and portable printers are provided for documentation and communication
- BayCare uses specific documentation and scheduling systems that candidates must be familiar with or trained on
This position is ideal for experienced home health RNs with strong clinical skills in cardiopulmonary care, wound care, IV therapy, and managing high-risk patients in a home setting.
