Registered Nurse (Behavioral Health) - Travel/Contract

Location: Clifton Springs, New York
Shift: 3 shifts per week, 12 hours each (7:00 PM - 7:30 AM)
Start Date: January 5, 2025
Weekend Requirement: Every other weekend
On Call: None
Contract Length: 13 weeks
License Requirement: Must have a valid New York RN license in hand
Travelers: First-time travelers accepted
Local Candidates: Must live more than 75 miles from the facility

Required Credentials & Certifications

• New York RN License (Primary source verification within 30 days of start)
• ACLS (Advanced Cardiac Life Support) – AHA provider/instructor/RQI acceptable
• BLS (Basic Life Support) – AHA provider/instructor/RQI acceptable
• PALS (Pediatric Advanced Life Support) – As required by unit
• NRP (Neonatal Resuscitation Program) – As required by unit
• TNCC (Trauma Nursing Core Course) – As required by unit
• EFM (Electronic Fetal Monitoring) – As required by unit
• NIH Stroke Certification – As required by unit, updated annually
• Tech Certification Verification – As required within 30 days of start
• Orientation Safety Manual Acknowledgement – Required
• NURSYS Verification (for RN, LPN/LVN, AP) – Within 30 days of start

Background & Compliance Requirements

• Government-issued photo ID (Driver’s License or State ID)
• 7-year criminal background check (felony checks) within 3 months of start
• EPLS/GSA/SAM check within 3 months of start, updated monthly
• HHS/OIG check within 3 months of start, updated monthly
• Education verification from primary source (no hard copy degrees accepted)
• Foreign equivalency education evaluation if applicable (CGFNS full report, WES, or EEE acceptable)
• Transcript/diploma if education verification not on file
• Employment application and work history verification (minimum 2 past employments verified)
• References (2 within 3 years) required for Aya candidates only
• Skills checklist required annually
• State-specific checks for New York including:

  • NYS Nurse Aide Registry (if applicable)
  • NYS Criminal Background Check Fingerprint Results (for Behavioral Health units)
  • Disciplinary Action Search (for Behavioral Health units)
  • Sex Offender Registry Search within 30 days of start
  • NY Medicaid Exclusion List check within 3 months of start, updated monthly

Health & Immunization Requirements (Must be completed within 5 days of contract signing)

• Physical exam within 1 year of start, updated annually
• Fit mask test within 1 year of start (Halyard duckbill 1870+1860 Regular or 1860 Small)
• Negative TB test (Qgold, Tspot, or 2-step PPD) within 1 year of start or negative chest X-ray if positive
• TB questionnaire annually (acceptable in lieu of annual TB test)
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (Surface AB positive titer, completed series, or declination)
  • Varicella (positive titer, 2 vaccines, or documented history)
• Drug screening within 30 days of start (includes amphetamines, cocaine, ecstasy, fentanyl, heroin, opiates)
• Seasonal flu vaccination or signed declination (deadline December 3)
• Eye exam (Snellen) within 1 year of start
• COVID-19 vaccination documentation preferred but not required

• First-time travelers are accepted for this assignment
• No on-call required
• Weekend shifts every other weekend
