Job Opportunity: Registered Nurse (Home Health)
Location: Portland, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Start Date: ASAP
- Shift: 8-hour day shifts only (no 4x10 or 3x12 shifts)
- Patient Ratio: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
- Experience Required: Minimum 1 year of skilled home health nursing experience
- First-Time Travelers: Accepted
- Weekend Requirement: None
- Certifications Required: BCLS (Basic Cardiac Life Support)
- On-Call: Not required
- Floating: Not required
- Block Scheduling: Not accommodated
- Locals Accepted: Yes (within 50-75 mile radius)
- Pending License: Accepted
- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Transportation: Must have own transportation
- Travel Radius: Up to 60 miles per day (60-90 minutes total drive time)
- COVID-19 Vaccination: Primary series plus one booster required
- Documentation System: EPIC Remote Client (laptop), Rover app (phone), and OASIS documentation required
- Attire: Business casual; scrubs allowed but no specific color required; no jeans
- Parking: Free
- Mileage Reimbursement: IRS standard
Required Qualifications & Certifications
- Basic Life Support (BLS) certification required
- Skilled Home Health Experience (minimum 1 year)
- COVID-19 primary vaccination series plus booster
- OASIS documentation experience preferred (please note if not experienced)
Compliance & Background Requirements
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Various state-specific background and abuse/neglect checks as applicable
- Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations and health screenings required:
- Tetanus/Diphtheria/Tdap (within 10 years or declination accepted)
- Negative TB test or chest X-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal Flu vaccine or declination (required during flu season)
- COVID-19 vaccine or declination
- Respirator clearance (RespSafety) required for travel candidates, updated annually
- Fit mask testing required for travel candidates, updated annually
Documentation & Training Requirements
- Employment application covering last 5 years (7 years for APN/Subcontractors)
- Minimum of 2 professional references covering 1 year of employment in relevant specialty
- Skills checklist updated annually (if applicable)
- Completion of Providence Health and Services WorkRamp training modules prior to start, including:
- Core modules for Oregon Home Health professionals
- Specialty-specific modules if applicable
- Annual regulatory requirements
- EPIC training modules required for Oregon and Spokane candidates, to be completed within one week of orientation
- Photo for badge (passport style with neutral background) required
Additional Notes
- Floating policy acknowledgment may be required (may float to other Providence facilities within reasonable distance)
- Facility requires primary series COVID vaccination plus booster
- Must have reliable transportation due to travel between patient homes
- Facility uses EPIC and Rover for charting and documentation
- Locals within 50-75 miles considered local and paid at same rate
