Required Certifications

• BLS (Basic Life Support): Required for all candidates (AHA provider or AHA RQI only)
• ACLS (Advanced Cardiac Life Support): Required for this RN position (AHA provider or AHA RQI only)
• Preferred Certification: NIHSS (National Institutes of Health Stroke Scale)

Background and Verification Requirements

• 7-year background check including aliases, sex offender, national, federal, criminal, FACIS III, education verification, and international checks if applicable.
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start.
• Education verification required for highest level of education (ADN or BSN for RN).
• Fingerprinting via Illinois Health Care Worker Registry required if no Illinois state license.
• NURSYS verification for RN licensure within 30 days of assignment start.
• Employment verification covering last 7 years.
• Skills checklist updated annually for assigned and float units.
• Two professional references required.

Health and Immunization Requirements (Time Sensitive)

• Physical exam within 1 year, updated annually.
• Fit mask test within 1 year (specific 3M masks required).
• Tetanus/Diphtheria/Tdap vaccination or declination (within 10 years).
• Negative TB test or chest X-ray within 1 year; TB questionnaire if positive.
• Proof of immunity or vaccination for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (declination allowed)
  • Varicella
• Drug testing: 9-panel minimum within 30 days of start, updated annually.
• Seasonal flu vaccination/documentation required for 2025-26 flu season; religious or medical exemptions require facility approval.

Facility Specific Requirements

• Photo for badge (passport style, plain background, forward facing, no headgear).
• Completion of COVID-19 and Influenza exemption forms if applicable (religious or medical).
• Compliance with facility-specific background attestation if applicable.

Additional Notes

• Candidates must have a current resume including the last 7 years of employment with explanations for any gaps over 30 days.
• Candidates must be able to float to other units as required.
• Illinois Adult Abuse Registry Search is not required for this facility.
• Fingerprinting must be completed in Illinois and within 10 days of authorization if required.

This position offers an opportunity to work in a med-surgical unit with a 1:6 patient ratio, requiring flexibility for floating and weekend shifts every other weekend. Certification and health documentation compliance is mandatory, with strict timelines for immunizations and background checks.
