Job Description

• Position: Inpatient Pharmacist
• Work Hours: Weeknights 9pm-7am, Weekend nights 7pm-7am
• Requirements:

  • PharmD degree
  • Licensed to practice pharmacy in North Carolina

Required Credentials & Documentation

Licensure & Certifications

• Pharmacy license valid in North Carolina
• ACLS (Advanced Cardiac Life Support) - ARC, AHA, or US Department of Defense approved
• BLS (Basic Life Support) - ARC, AHA, or US Department of Defense approved
• Other certifications as applicable (e.g., PALS, NRP) depending on facility/unit requirements

Background Checks

• 7-year criminal background check within one year of start, updated annually
• Must include all locations lived/worked in past 7 years
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for renewals
• Completion of Criminal History Investigation Form if background check returns a hit

Health & Immunization Requirements

• Initial physical exam, updated annually
• Negative TB test or chest X-ray within 1 year, updated annually
• Immunizations or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Flu vaccination or approved medical/religious exemption required during flu season (Oct 1 - Apr 30)
• 9-panel drug screen within 90 days of start, updated annually

Competency & Testing

• Medication Test/Pharmacology Competency Exam as applicable
• Core competency exams required
• Facility-specific orientation and post-tests must be completed

Facility-Specific Requirements

• Passport-style photo for badge (shoulders up, front facing, plain background) submitted before start
• Completion of self-study orientation and accountability statements
• Submission of personal data sheet
• Compliance with facility-specific policies and documentation

Additional Notes

• Proof of 2 clinical references required
• Performance evaluations required if previously worked with the system over 1 year
• Employment application/resume must be complete
• Skills checklist updated annually for applicable units
