Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits with periodic office meetings.
- Average patients per day: 5-8 (assigned 5-10 patients daily, equating to roughly 6-8 points).
- Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
- Must have reliable transportation, current driver's license, and agency-required liability insurance.
- Must be willing to travel up to 75 miles from the home clinic; average weekly travel is 150-200 miles.
- Work schedule: Monday to Friday, 8 AM to 5 PM. No weekend rotation or on-call required.
- Territory: Sussex County, Delaware.
- Charting: Proficiency required in HomeCare HomeBase (HCHB) used within the last 6 months; retraining required if last use was over 6 months ago. RNs must also be proficient in OASIS.
- EMR system used: PointClickCare.
- Experience required: Minimum 1 full year of recent Adult Home Health or Hospice experience (depending on specialty). First-time travelers not accepted.
- Locals accepted at the same rate.
- Some positions may have a Baylor Shift schedule (4x10s, Friday-Monday), but most are standard 5x8s.
Requirements
- Licensure: Must have an active nursing license in hand; pending licenses not accepted.
- Certifications: Current CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
- Experience: Minimum 1 year recent specialty experience in Adult Home Health or Hospice.
- Transportation: Reliable transportation and current driver's license required.
- Mileage: Reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not billable).
Compliance & Documentation
Candidates must complete and maintain the following prior to start and during assignment:
Background Checks & Verifications
- Standard 7-year background check including SS trace, county, nationwide, sex offender, and motor vehicle record.
- EPLS/GSA/SAM and HHS/OIG checks within 14 days of start, updated monthly.
- State-specific background checks as required by assignment location.
- Education verification for applicable degree.
- References: Two references required for all placements.
- Work history verification: At least two most recent employers or last 7 years if only one employer.
Health & Immunizations
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- Drug screening: 10-panel drug screen required.
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination or documented declination as per state policy.
Facility-Specific Documents & Training
- Personal or auto insurance policy under candidate’s name or listing candidate as a driver.
- Review and sign various compliance documents including:
- NDA and End User Agreement
- Compliance Basics and Practice Quiz
- Code of Conduct
- HIPAA and Privacy Policy
- Confidentiality Form and Covenant
- Home Health Scenarios and Core Policy Acknowledgements
- Hospice Core Care Policy Quiz (if applicable)
- Employee Responsibility Agreement
- COVID-19 Policy and Attestation
- TB Questionnaire and Risk Assessment (if applicable)
- Other state-specific forms and attestations as required.
Training
- Proficiency in HomeCare HomeBase (HCHB) and OASIS required; if not current, must complete retraining.
- OASIS E-Course required for RNs and certain therapists without recent HCHB experience.
Additional Notes
- This position is created due to RN resignation and LPN contractor end date.
- No guaranteed hours; if patient census is low, floating to nearby clinics may be discussed.
- Dress code: Scrubs.
This opportunity is ideal for experienced Registered Nurses with recent home health or hospice experience, comfortable with travel within Sussex County, Delaware, and proficient in required EMR systems and documentation processes.
