Physical Therapist (PT)

Job Overview

• Primarily outpatient setting with some acute care coverage.
• Delegates responsibilities to Physical Therapist Assistants and Aides.
• Focus on patient assessment, planning, intervention, and evaluation following physical therapy standards and physician orders.

Key Responsibilities

• Evaluate patient function using formal and informal testing to assess skeletal, neurological, respiratory, and cardiovascular status.
• Develop and update goal-specific treatment plans based on patient assessment and tolerance.
• Coordinate treatment plans with physicians and clinical support staff.
• Educate patients, families, and caregivers on conditions, prognosis, and treatment rationale.
• Supervise Physical Therapist Assistants, Aides, Home Health Aides, and students according to regulatory standards.
• Recommend and assist in securing adaptive/supportive equipment for patients.
• Document patient responses and changes in condition in compliance with regulatory and affiliate policies.
• Provide treatment interventions to restore or facilitate optimal functioning.
• Participate in team conferences, interdisciplinary meetings, and program development.
• Promote physical therapy services within the community and organization.
• Function as case manager for assigned cases.
• Maintain confidentiality and adhere to organizational policies and procedures.

Physical and Mental Requirements

• Medium physical demand level: occasional lifting/carrying 21-50 lbs, frequent lifting of 20 lbs, and constant handling of 10 lbs.
• Ability to sit, stand, walk for long periods, and perform postural changes (stooping, kneeling, crouching).
• Exposure to communicable diseases and bloodborne pathogens; must follow safety protocols.
• Ability to wear personal protective equipment correctly.
• Good overall vision and hearing (with or without assistive devices).
• Ability to handle multiple tasks, work independently, and maintain regular attendance.
• Effective verbal and written communication skills in English.

Required Certifications and Education

• BLS (Basic Life Support) certification within 1 month of hire or transfer.
• Licensed Physical Therapist (PT).
• Bachelor’s Degree required.

Experience and Skills

• Successful completion of facility-specific orientation and new hire competency.
• No explicit years of experience stated; however, this is a licensed PT role requiring professional competency.
