Position Details

Required Certifications

• CNMT (Certified Nuclear Medicine Technologist)
• State License (Illinois)
• ACLS, BLS, PALS as required by unit/specialty (AHA or RQI acceptable)
• Primary Source Verification of certifications and licenses

Compliance & Documentation Requirements

• Valid US Driver’s License or State ID
• Background Checks (within 30 days of start):

  • SSN Trace, Counties, States, National Criminal, National Sex Offender, FACIS Level 3
  • EPLS/GSA/SAM
  • HHS/OIG
• Fingerprinting: Illinois Health Care Worker Registry Search (must be done in Illinois 3 business days prior to start)
• Education Verification (degree verification required)
• Competency Exam (required for all clinical/allied candidates)
• Photo for badge (color photo, mid-shoulders up, plain background)
• Code of Conduct acknowledgment
• Parking Permit Form (time sensitive)
• Badge Name confirmation

Health & Safety Requirements

• Physical Exam (within 1 year of start) confirming good health and ability to perform job functions
• Fit Mask Test (annual expiration required for new contracts)
• Negative TB test or Chest X-Ray (within 1 year)
• TB Questionnaire (updated annually)
• Immunizations or documentation for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug Testing: 9-panel drug screen within 30 days of start
• Flu Vaccination or approved Medical/Religious Exemption required annually

• Two professional references required
• Skills Checklist required for clinical/allied candidates

This position offers guaranteed hours with some allowance for client call-offs during the assignment. Candidates should be prepared to meet all licensure, certification, and health compliance requirements prior to start.
