Job Opportunity: Digital Acute Care Registered Nurse (SFMC - On Call)

Location: Peoria, Illinois

Position Summary

• Provide virtual and in-person nursing care to adult patients using telehealth technology and hands-on care in patients' homes.
• Monitor, prevent, and intervene early to optimize patients' quality of life at home.
• Use evidence-based care and clinical decision support tools.
• Collaborate with healthcare team members, patients, and families to promote quality care.
• Act as a patient advocate and resource for healthcare providers and community resources.
• Demonstrate service excellence and accountability, balancing cost-effective and efficient care delivery.

Required Qualifications

• Education: Associate Degree in Nursing
• Licenses/Certifications:

  • Registered Nurse (RN) license
  • American Heart Association BLS certification
  • Valid current Illinois state driver's license
• Experience:

  • Minimum 1 year recent RN experience in adult acute care population
  • Minimum 2 years experience in roles requiring effective communication and collaboration with healthcare professionals
• Skills:

  • Excellent interpersonal and communication skills
  • Proficient computer skills, including Microsoft software
  • Strong analytical and problem-solving abilities with attention to detail

Preferred Qualifications

• Bachelor’s Degree in Nursing
• Proficiency with electronic medical records (EMR) and communication platforms

Additional Requirements

• Must be able to travel to patient homes within a 30-minute radius of the OnCall building (fleet car provided)
• Compliance with motor vehicle report and insurance coverage requirements (for homecare/driving roles)
• Transportation for travel to/from patients' homes
• Drug-free workplace: must produce a negative drug screen for all panels

Notes

• Orientation includes 12 non-billable hours
• Call off policy allows 12 hours off every 13 weeks without penalty
• Local rate adjustments apply for clinicians living within 50 miles of the facility
