Job Opportunity: Digital Acute Care Registered Nurse (SFMC - On Call)

Position Summary

• Provide virtual and in-person nursing care to adult patients across the Ministry using telehealth technology and hands-on care in patients' homes.
• Responsible for proactive monitoring, prevention, early intervention, and ongoing care to optimize patients' quality of life at home.
• Utilize evidence-based care and clinical decision support to guide nursing practice.
• Collaborate with healthcare team members, patients, and families to promote quality care.
• Act as a patient advocate and resource for other healthcare providers and community resources.
• Demonstrate service excellence and accountability, balancing cost-effective and efficient care delivery.

Required Qualifications

• Education: Associate Degree in Nursing
• Licenses/Certifications:

  • Registered Nurse (RN) license
  • American Heart Association BLS certification
  • Valid current Illinois state driver's license
• Experience:

  • Minimum 1 year recent RN experience in adult acute care population
  • Minimum 2 years experience in roles requiring effective communication and collaboration with healthcare professionals
• Skills:

  • Excellent interpersonal and communication skills
  • Proficient computer skills, including Microsoft software
  • Strong analytical and problem-solving abilities with attention to detail

Preferred Qualifications

• Bachelor’s Degree in Nursing
• Proficiency with electronic EMR and communication platforms
• Reliable transportation for travel to/from patients' homes within a 30-minute radius (fleet car provided)

Additional Information

• Must comply with motor vehicle report and insurance coverage requirements for homecare/driving roles.
• Travel involves visiting patient homes within a 30-minute radius of the OnCall building.
• Orientation includes 12 non-billable hours.
• Drug-free workplace: negative drug screen required for all panels.
