Job Details

• Start Date: ASAP
• Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
• Patient Ratio: 1:1 (May visit up to 6 patients per day; 30 productivity points standard)
• Experience Required: Minimum 1 year of skilled home health experience
• First-Time Travelers: Accepted
• Weekend Work: Not required
• On-Call: Not required
• Floating: Not required
• Block Scheduling: Not accommodated
• Locals Accepted: Yes (within 50-75 mile radius)
• Pending License: Accepted
• Transportation: Traveler must have their own transportation
• Travel Radius: Up to approximately 60 miles per day (60-90 minutes total drive time)
• Documentation System: EPIC Remote Client (laptop), Rover (phone app), and Oasis
• Oasis Experience: Required
• Guaranteed Hours: Facility may call off up to one shift every two weeks
• Attire: Business casual; scrubs allowed but no specific color required; no jeans

• Mileage Reimbursement: IRS standard

Required Certifications

• BCLS (Basic Cardiac Life Support) - Must be current and from AHA or Canadian Heart & Stroke Foundation
• Other certifications may be required based on clinical track but not specified for this role

Experience Requirements

• Specialty Experience: Minimum 1 year of skilled home health experience
• Prior Travel Experience: Not required

Compliance and Documentation Requirements

• Background Check: Must be completed within 45 days of start, covering all locations lived/worked in the past 7 years; includes SSN trace, death master index, nationwide checks, MVR (required for home health), and state-specific abuse/neglect searches as applicable
• Education Verification: Required for all candidates
• Employment Application: Must include at least last 5 years of employment with month/year format; gaps or overlaps must be explained
• References: Minimum 2 supervisor references covering 1 year of employment within the last 3 years in a comparable specialty/setting
• Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work with or without accommodation
• Drug Testing: 10-panel drug screen within 90 days of start (instant tests not accepted)
• Immunizations and Health Documents:

  • TDAP (within 10 years) or declination
  • Negative TB test or chest X-ray within 12 months
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
  • Hepatitis B vaccination or signed declination
  • Varicella vaccination or positive titer or declination
  • Flu vaccine during flu season or declination
  • COVID vaccine or declination
• Respiratory Safety Clearance: Required for travel candidates, must be medically cleared annually for respirator use
• Fit Mask Testing: Required for travel candidates, must be updated annually; onsite testing available if needed

Facility and System Requirements

• EPIC Training: Completion of EPIC modules required within a week of orientation
• WorkRamp Modules: Completion of required pre-start, specialty-specific, and annual regulatory training modules via WorkRamp LMS platform
• Oregon and Washington State Specific:

  • Oregon Abuse/Neglect Search (if applicable)
  • Washington WATCH and WSP State Patrol Searches for border facility placements
  • Washington OBRA Registry Search for CNAs (not applicable for PTA)
• Float Policy: Acknowledgement required; may be asked to float to other Providence facilities within reasonable distance
