Job Opportunity: RN ICU - Waterloo, Iowa
Position Details
- Specialty: RN ICU

- Schedule: Every Third Weekend
Required Experience

- Trinity 10 Panel Drug Screen (completed within 30 days of start)
- COVID-19 Vaccine (including booster by Sept 2025, or approved declination/exemption)
- MMR (Proof of 2 vaccinations or IgG titer)
- Hepatitis B (Vaccination series required; titers not accepted)
- Influenza Vaccine (Proof or declination for 2024-2025 season)
- 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
- Varicella (Proof of 1 vaccination or IgG titer)
- Physical/Health Statement (Exam within last year confirming fitness to work)
- Tdap Vaccine (Current and non-expired; declination accepted if no pediatric interaction)
Professional & Compliance Requirements
- Trinity Profile (Resume, Skills Checklist within a year, 2 Professional References)
- Trinity Code of Conduct Form
- Trinity I9 Verification
- Confidentiality and System Security Agreement
- Integrity and Audit Services Form
- Non-Employee Upload Form
- Specialty Specific Competency Testing (Must be completed within a year)
- Conflict of Interest Disclosure Form
- Criminal Background Check (Federal, Nationwide, County for last 7 years; SSN verification)
- Education Verification (Highest academic degree or diploma/GED)
- EPLS (Excluded Parties List System) Check
- GSA/OIG (Office of Inspector General) Check
- Joint Commission CORE Exam (within a year)
- NSO (National Sex Offender) Check (within 30 days of start)
- System Access Form (Completed and uploaded one week before start)
