Job Details

Certifications Required
- BLS (Basic Life Support): Required for all candidates (AHA provider or AHA RQI accepted; RQI provider like Laerdal not accepted)
- ACLS (Advanced Cardiac Life Support): Required for specific RN/Unit positions (AHA provider or AHA RQI accepted)
- PALS (Pediatric Advanced Life Support): Required for specific RN/Unit positions (AHA provider or AHA RQI accepted)
- NRP (Neonatal Resuscitation Program): Recommended for Nursery, NICU, and L&D
- EFM (Electronic Fetal Monitoring): AWHONN Intermediate recommended for L&D
- TNCC (Trauma Nursing Core Course): Recommended for ER
- State License Verification: Must be verified through state licensing board within 30 days of assignment start
- NURSYS Verification: Required within 30 days of assignment start for RNs and LVNs
Background and Compliance Requirements
- 7-year background check including aliases, sex offender, national and federal criminal checks, FACIS III, education verification, and international checks if applicable
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
- Fingerprinting required for candidates without an Illinois state license; must be completed in Illinois and added to the Illinois Department of Public Health (IDPH) Registry
- Education verification required for highest level of education; diploma/transcripts acceptable if verification unavailable
- Drug testing: 9-panel minimum within 30 days of start, updated annually (includes cocaine, PCP, amphetamines, opiates, benzodiazepines, barbiturates, methadone, propoxyphene, methaqualone)
- References: 2 required
- Employment application including last 7 years of employment with explanations for gaps longer than 30 days
- Work history verification for 7 years required, to be completed 2 Fridays prior to start date
Health and Immunization Requirements
- Physical exam within 1 year, updated annually, confirming good health
- Fit mask test within 1 year of start (specific 3M masks required)
- Tetanus/Diphtheria/Tdap vaccination within 10 years or declination (Td not accepted in lieu of Tdap)
- Negative TB test (PPD, Qgold, or Tspot) within 1 year and updated annually; clear chest X-ray required for positive TBs
- TB questionnaire required for positive TBs
- Proof of immunity or vaccination for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (declination allowed)
- Varicella
- Flu seasonal vaccination/documentation required for 2025-26 season (10/1/25 to 3/31/26); religious or medical exemptions require facility approval; declinations not accepted
Facility Specific Requirements
- Passport-style photo with plain background required for badge
- COVID-19 and Influenza religious and medical exemption forms must be completed and approved at least one week prior to documentation due date if applicable
- Compliance checklist and background attestation documents required for audit and verification purposes
