Registered Nurse - Home Health (Travel/Contract)

Location: Marlborough, Massachusetts
Contract Length: 13 weeks


Job Description

  • Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Conduct home visits with periodic office meetings.
  • Average patient load: 5-8 patients per day.
  • Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
  • Must be willing to travel up to 75 miles from home clinic; weekly travel estimated between 50-200 miles.
  • Schedule: Monday to Friday, 8 AM to 5 PM; some weekend rotation possible but not mandatory.
  • On-call: Not required for this assignment.
  • Charting: Proficiency required in HomeCare HomeBase (HCHB) used within the last 6 months; RNs must also be proficient in OASIS.
  • Dress code: Scrubs.

Requirements

  • Specialty: Registered Nurse with Home Health or Hospice experience.
  • Years of Experience: Minimum 1 full year of Adult Home Health experience or 1 full year Hospice experience (whichever applies to specialty).
  • Prior Travel Experience: Not required.
  • Certifications: Current CPR and BLS certification required.
  • Licensure: Must have an active RN license in hand; pending licenses are not accepted.
  • Transportation: Reliable transportation, current driver's license, and agency-required liability insurance required.
  • First-timers: Not accepted; must have recent specialty experience.

Productivity Expectations

  • Weekly productivity units: 32 points.
  • Patient visits and activities are assigned point values; typically 5-10 patients per day equate to 6-8 points daily.

Additional Information

  • Local candidates accepted at the same rate.
  • Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8 Monday-Friday.
  • Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).

Required Compliance and Documentation

  • Must complete all required background checks, including but not limited to:
    • 7-year standard background check with aliases, SS trace, county, nationwide, sex offender, and state-specific searches.
    • EPLS/GSA/SAM and HHS/OIG screenings.
    • Fingerprinting as required by state regulations.
  • Required health documents:
    • Physical exam within 12 months prior to start.
    • Negative TB test or chest X-ray within 12 months, updated annually.
    • Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
    • Drug screening (10-panel).
    • Seasonal flu vaccination or documented declination.
    • COVID-19 vaccination per Massachusetts state policy (full vaccination required, no declinations accepted).
  • Must review and sign facility-specific compliance documents including:
    • Code of Conduct, Confidentiality Forms, HIPAA and Privacy Policies, Compliance Training, and other home health/hospice-specific policies.
  • Must verify education and work history (minimum 2 most recent employers or last 7 years).
  • Two professional references required.

Summary

This is a travel Registered Nurse position in home health care requiring at least 1 year of recent specialty experience in adult home health or hospice. The role involves patient visits in the community with travel up to 75 miles from the home clinic, proficiency in specific EMR systems, and adherence to comprehensive compliance and health documentation requirements. Weekend work may be required occasionally but is not mandatory, and on-call duties are not part of this assignment.
