- Fingerprinting: Illinois Healthcare Worker Registry search required for clinical candidates without an IL state license; prints must be taken in Illinois 3 business days prior to start (not required for non-clinical or remote candidates)
- Education Verification: Initial degree verification required; unofficial or official transcripts accepted if verification is pending
- State-specific searches (Illinois Department of Corrections, State Police Murderer and Violent Offender Against Youth Registry, Most Wanted) required within 30 days of start

Health & Safety Requirements
- Physical Exam: Required within 1 year of start; must confirm good physical and mental health, free of communicable disease, and ability to perform job functions without restrictions

- Negative TB or Chest X-Ray: 2-Step PPD, TSPOT, or Qgold within 1 year of start; if positive TB test, chest X-ray and documentation required

- Immunizations:
- Rubeola (Measles): 2 MMR vaccines, positive titer, or signed declination
- Mumps: 2 MMR vaccines, positive titer, or signed declination
- Rubella: 2 MMR vaccines, positive titer, or signed declination
- Hepatitis B: Vaccine series, positive titer, or signed declination
- Varicella: 2 vaccines, positive titer, or signed declination
- Drug Testing: 9-panel drug screen within 30 days of start; negative dilute results require retest; not required for non-patient facing or remote candidates
- Flu Vaccination or Medical/Religious Exemption: Required by 11/30 annually; declinations not accepted; not required for non-patient facing or remote candidates
