Required Exams and Competencies

• Medication Test/Pharmacology Competency Exam (required prior to start)

• Dysrhythmia/EKG Competency Exam (required within 1 year of start and updated annually)

Background and Credentialing Requirements

• 7-Year Criminal Background Check within 90 days of start (including FBI/BCI fingerprinting)
• Monthly EPLS/GSA/SAM and HHS/OIG checks
• Education Accreditation Confirmation from an accredited nursing program (CCNE, ACEN, NLN CNEA, or CASN)
• Transcript/Diploma verification
• Fingerprinting (Ohio BCI & FBI) required within 1 year of start; must be completed using specific codes and processes
• Pending Background Check Attestation required if fingerprint results are pending

Health and Immunization Requirements

• Physical Exam within 1 year of start confirming good health and free of contagious disease
• Tetanus/Diphtheria/Tdap (adult dose after age 11/12)
• Negative TB test or chest X-ray within 1 year of start; TB questionnaire required annually
• MMR (Measles, Mumps, Rubella): 2 vaccines or positive titer; boosters required if low titer
• Hepatitis B titer or 3-vaccine series (declination acceptable)
• Varicella: 2 vaccines or positive titer; medical exemptions reviewed
• Negative 10 Panel Drug Screen within 3 months of start (includes Amphetamines, Benzodiazepines, Cocaine, Opiates, etc.)
• Seasonal Flu Vaccination or declination during flu season (10/1 - 3/31); mask required if declining
• COVID-19 vaccination not required but strongly recommended

• 2 professional references required (at least 1 from a manager or formal leadership)
• Employment application and skills checklist required
• Annual Orientation Safety Manual Acknowledgement
• Various facility-specific compliance and ethics policies must be reviewed and signed annually
• Fraud, Waste, and Abuse Training Certificate required prior to start and updated annually

• No Return to Office (RTO) between 2/15/26 and 2/28/26 due to new tower opening
• Facility uses EPIC for documentation
• Candidates must be prepared for a 1:4 staffing ratio
