Position Details

Licenses & Certifications

• BLS: American Heart Association or Red Cross certification required
• Nursys: License verification required
• Professional License/Certificate Verification: Primary source verification within 30 days of start
• Driver's License/State ID/Passport: Required for onboarding

Exams & Testing

• Prime Basic Pharmacology Exam: Must score 80% or higher
• Prime Core Competencies Exam: Must score 80% or higher
• Prime Skills Checklist: Required
• Prime Specialty Exam: Must score 80% or higher

Background & Compliance

• Fingerprinting: Required (using IdentoGo)
• Criminal Background Check: National check within one year of start date; specific state exceptions apply
• SAM/EPLS Exclusion Check: Must be completed and dated

Health & Immunizations

• COVID-19 Vaccine: 2 doses plus booster or completed declination
• MMR: Proof of 2 vaccines or positive titer
• Hepatitis B: Required (declinations not accepted at some facilities)
• Influenza Vaccine: Required or declination during flu season (Nov 1 - Mar 31)
• TB Test: Within one year of start date
• Varicella: Vaccination or titer showing immunity
• Tdap: Required (declinations not accepted at some facilities)
• Fit Test: Annual respirator fit test required prior to start or onsite if available
• Physical/Health Statement: Annual clearance to work required within 60 days of start

Additional Requirements

• Drug Screening: Minimum 10-panel test including THC within 60 days of start
• Orientation Packet: Completion of essentials form and post-test
• Job Description: Signed by employee
• Education Verification: Diploma or transcript required
• Workplace Violence Prevention Training (DCI/BVP): Required, can be completed via Relias or similar platforms
• Badge Photo: Clear JPEG headshot with plain background required
• Agency Attestation: Completed after all credentials are uploaded
