Location: Fostoria, Ohio

Job Details

  • Work Schedule:

    • Friday hours: 8:00 AM - 12:00 PM
    • No weekends required
  • Patient Population:

    • Pediatrics, Adolescents, Adults, Geriatrics
    • Average Daily Census: 8 patients
    • Rooms/Beds: 20 each
  • Staffing Ratio: 1:1

  • Patient Diagnoses: Variable

  • Special Procedures:

    • Videofluoroscopic Swallow Study (VFSS)
  • Work Settings:

    • Acute Care

  • Dysphagia Management:

    • Bedside Swallow Evaluation
    • Conducting and Interpreting Modified Barium Swallow Studies

  • Electronic Medical Record: Epic required

Required Certifications and Licenses

  • CCC-SLP (Certificate of Clinical Competence in Speech-Language Pathology)
  • MBSImP (Modified Barium Swallow Impairment Profile)
  • Ohio State License

Experience Requirements

  • Fingerprinting clearance attestation required for specific units

Health and Immunization

  • Physical exam within last 12 months (latex-free PPE available)
  • N95 Fit Test within 12 months, updated annually (KN95 not accepted)
  • Tetanus/Diphtheria/Tdap within 10 years or declination accepted
  • Negative TB test (PPD, QGold, or TSPOT) within 3 months, updated annually
  • MMR (Measles, Mumps, Rubella) - positive titer, 2 vaccines, or approved exemption required
  • Hepatitis B series, positive titer, or declination accepted
  • Varicella - positive titer, 2 vaccines, or approved exemption required
  • Negative 10-panel drug screen within 12 months (instant acceptable unless on prescription)
  • Seasonal Flu vaccination or approved exemption required during flu season (10/1 start, documentation deadline 11/1)

Certifications (as applicable)

  • ACLS, BLS, PALS, NRP, TNCC, EFM, NIH Stroke Certification, CPI (Crisis Prevention Institute) - check unit-specific requirements
  • Medication/Pharmacology competency exams as applicable
  • Dysrhythmia/EKG competency exam required for RNs (not applicable here)

Other Requirements

  • 2 professional references within 2 years, at least one from a leader (Charge RN or above)
  • Skills checklist required for all candidates
