Registered Nurse - Home Health (Travel/Contract)

Location: Augusta, Georgia
Contract Length: 13 weeks

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits and periodic office meetings.
• Average patients per day: 5-8.
• Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
• Must be willing to travel up to 75 miles from home clinic; travel between patient homes is compensated.
• Schedule: Monday to Friday, 8 AM to 5 PM (some positions may have 4x10 shifts Friday-Monday).
• Visits occur both in patient homes and care facilities.
• On-call requirement: Approximately 1 weekend per month (varies by location).
• Weekend requirements vary by location and profession; discuss during interview.
• Local candidates accepted at the same rate.

Requirements

• Specialty: Registered Nurse (Home Health)
• Experience: Minimum 1 full year of Adult Home Health experience OR 1 full year Hospice experience (whichever is higher).
• First-timers: Not accepted; must have recent specialty experience.
• Certifications: Current CPR and BLS required.
• Licensure: Must have license in hand; pending licenses not accepted.
• Transportation: Reliable transportation and current driver's license required. Agency-required liability insurance also required.
• EMR Systems: Proficiency with HomeCare HomeBase (HCHB) within the last 6 months required; retraining required if last use was over 6 months ago. RNs must also be proficient in OASIS.
• Dress Code: Scrubs.

Productivity Expectations

• Weekly units of productivity: 30
• Weekly travel: Approximately 30-40 miles per day

Compliance & Documentation Requirements

• Must complete various background checks including:

  • 7-year standard background check (including SS trace, county, nationwide, sex offender, state-specific searches).
  • EPLS/GSA/SAM and HHS/OIG checks within 14 days of start.
  • Fingerprinting as required by state (varies by location).
• Education verification required.
• Drug testing: 10-panel drug screen required.
• Health documents required:

  • Physical exam within 12 months.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
  • Flu vaccination or documented declination for current season.
  • COVID vaccination series or declination.
• References: Two references required.
• Work history verification: At least two most recent employers or last 7 years if only one employer.

• Candidates must review and sign various compliance, confidentiality, and policy documents prior to start.
• Candidates must complete required state-specific forms and attestations depending on assignment location.
• Candidates must be prepared for ongoing annual compliance requirements if working consecutively for more than one year.

This position is ideal for experienced Registered Nurses with home health or hospice experience who are comfortable with travel and working independently in patient homes and care facilities.
