Nursing Contract Opportunity - Vermont (Rutland)

Position Overview

  • Specialty: Nursing (Home Health / Hospice focus)

Key Requirements

  • OASIS experience required
  • Must be knowledgeable and compliant with:
    • Evidence-based nursing practices
    • State and Federal regulations
    • Vermont State Board of Nursing standards
  • Ability to serve as Case Manager, managing caseloads and visit times efficiently
  • Skilled in completing OASIS documentation accurately and timely
  • Proficient in implementing medical plans of care and nursing interventions
  • Experience with high-tech nursing skills such as IV use and maintenance, wound VAC, and complex wound care
  • Ability to work collaboratively with LPNs and other healthcare team members
  • Strong documentation skills to maintain accurate nursing care records
  • Capable of patient and caregiver education on medication, treatment options, discharge instructions, and other relevant topics
  • Experience coordinating interdisciplinary care and communicating patient needs to other disciplines (e.g., physical therapy, social work)
  • Ability to complete various agency, state, and federal forms timely (e.g., HHABN, Medicare Notice of Non-Coverage)
  • Skilled in coordinating safe patient discharges and transfers
  • Participation in peer review audits and case conferences
  • Advocacy for patient care sensitive to individual patient and family needs
  • Active participation in interdisciplinary team (IDT) meetings and care planning
