Job Details

• Contract Length: 13 weeks
• Shift: Days (5 shifts per week, 8-hour shifts from 8:00 AM to 4:30 PM, rotating weekends)
• Specialty: Registered Nurse (RN) - Care Management
• Patient Ratio: Approximately 1:25
• Environment: Fast-paced

Requirements

• Active Florida state or multistate RN license
• BLS certification
• Minimum of 2+ years hospital care management experience
• Experience with Cerner EMR system
• Ability to manage discharge planning for all levels of post-acute care
• Must be able to handle a high patient ratio and fast-paced environment

Responsibilities

• Collaborate with interdisciplinary healthcare team to meet resource management goals
• Assist in reducing physician practice variations
• Enhance patient outcomes with focus on decreasing length of stay
• Monitor cost-effective patient care across the continuum
• Determine medical necessity of admissions and continued stays
• Identify inappropriate or untimely services
• Assess discharge planning needs for patients of all ages
• Coordinate delivery of services to meet discharge needs
• Facilitate timely and appropriate hospital discharges
• Identify opportunities for patient care improvement
• Provide documentation as required by regulatory agencies
