Certifications Required

• BLS (Basic Life Support) - AHA provider or AHA RQI only (No other providers accepted)
• ACLS (Advanced Cardiac Life Support) - Required for this RN position; AHA provider or AHA RQI only
• Certifications Preferred: NIHSS (National Institutes of Health Stroke Scale)

Health & Compliance Requirements

• Physical Exam: Required within 1 year and updated annually
• Fit Mask Test: Required within 1 year of start (3M 1860 S/R, 3M 1870+, or 3M 9205+)
• Immunizations & Titers: Must be completed within 2 business days of contract signing

  • Tetanus/Diphtheria/Tdap (Tdap required, TD not accepted)
  • Negative TB test or Chest X-Ray (within 1 year)
  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (declination allowed)
  • Varicella
• Drug Testing: 9-panel minimum within 30 days of start, updated annually
• Flu Seasonal Vaccination/Documentation: Required for flu season (10/1 to 3/31); religious or medical exemptions require facility approval

Background & Verification Requirements

• Background Check: 7-year check within 30 days, including aliases and multiple databases
• Education Verification: ADN or BSN required; diploma/transcripts accepted if verification unavailable
• Employment Verification: 7 years of work history required; gaps over 30 days must be explained
• References: Minimum 2 references required
• Skills Checklist: Required annually for assigned and float units
• State License Verification: Must be verified within 30 days of assignment start
• NURSYS Verification: Required within 30 days of start for RNs
• Fingerprinting: Required only if no Illinois state license; must be done in Illinois and added to IL Department of Public Health Registry

Additional Requirements

• Photo for Badge: Passport-style photo with plain background, forward facing, no headgear
• COVID-19 & Influenza Exemption Forms: Must be completed and approved at least one week prior to documentation due date if applicable
