Job Opportunity: Surgical Services (Travel/Contract)

Location: Harvey, Illinois
Contract Length: 13 weeks

Job Requirements

• Specialty: Surgical Services
• Experience: CSFA with Labor & Delivery (L&D) and Obstetrics (OB) experience required
• Call Coverage: 24-hour call coverage on weekends

Certifications Required

• BLS (Basic Life Support): Required for all candidates (AHA provider or AHA RQI accepted; RQI provider like Laerdal not accepted)
• ACLS (Advanced Cardiac Life Support): Required for specific RN/Unit positions (AHA provider or AHA RQI accepted)
• PALS (Pediatric Advanced Life Support): Required for specific RN/Unit positions (AHA provider or AHA RQI accepted)
• NRP (Neonatal Resuscitation Program): Recommended for Nursery, NICU, and L&D
• EFM (Electronic Fetal Monitoring): AWHONN Intermediate recommended for L&D

Licensure & Verification

• State license primary source verification must be completed within 30 days of assignment start
• NURSYS verification required within 30 days of start for RNs and LVNs
• Illinois Department of Public Health (IDPH) fingerprinting required for candidates without an Illinois state license (must be done in Illinois)

Background & Compliance Checks

• 7-year background check including aliases, sex offender, national and federal criminal checks, FACIS III, education verification, and international checks if applicable
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
• Education verification required for highest level of education (ADN or BSN for RN positions)
• Drug testing: 9-panel minimum within 30 days of start, updated annually (includes cocaine, PCP, amphetamines, opiates, benzodiazepines, barbiturates, methadone, propoxyphene, methaqualone)
• References: 2 required
• Employment verification: 7 years required, with explanations for gaps over 30 days
• Skills checklist: Required and must be updated annually

Health & Immunization Requirements

• Physical exam within 1 year, updated annually
• Fit mask test within 1 year of start (specific 3M masks required)
• Tetanus/Diphtheria/Tdap vaccination within 10 years or declination (Tdap required)
• Negative TB test or chest X-ray within 1 year, updated annually; TB questionnaire required if positive TB
• Immunizations or proof of immunity required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (declination allowed)
  • Varicella
• Flu seasonal vaccination/documentation required for flu season (10/1 to 3/31); religious or medical exemptions require facility approval

Additional Requirements

• Photo for badge: Passport-style, plain background, forward-facing, no headgear
• COVID-19 and Influenza exemption forms (religious and medical) must be completed and approved at least one week prior to documentation due date if applicable
• Prior travel experience: Not explicitly required for this position
