Job Opportunity: RN Hospice - Home-Hospice Care Operations

Position Details

• Employment Type: Contractor
• Contract Length: 13 weeks
• Work Schedule: Weekend program, every Saturday and Sunday from 8 AM to 8 PM
• Work Setting: Primarily remote/field work in the Milwaukee territory, visiting patients in their homes, Assisted Living Facilities, Group Homes, Nursing Homes, and occasionally inpatient hospice settings.

Patient Population

• Hospice patients with life expectancy of 6 months or less if disease runs its normal course.
• Common diagnoses: approximately 40% cancer, 60% other medical issues.

Caseload and Visits

• RN caseload: 13-15 patients
• Visits per day: 5-6 visits per 8-hour shift

Experience Requirements

• RN Experience: Minimum 2 years of RN experience required (except for one inpatient hospice facility which accepts 1 year).
• Prior Travel Experience: Not required; first-time travelers are accepted.

Certifications Required

• Current state RN license
• BLS certification

Skills and Competencies

• IV skills, chest tube maintenance/pleurex drain, catheter insertion, tube feedings
• Triage skills, wound care, wound vac management
• Comfortable managing PIV, midline, central lines
• Competency in end-of-life care including pain and symptom management (pharmaceutical and non-pharmaceutical)
• Experience with opioids and opioid equianalgesic dosing
• Documentation proficiency in EMR platforms
• Independent critical thinker and problem solver with good judgment on when to seek assistance
• Flexibility to work in both field and inpatient settings
• Familiarity with palliative sedation (in inpatient facility)

Technology and Equipment

• EMR systems: EPIC Hyperspace/ClinDoc or Remote Client depending on role
• IV pumps: PCA pump, Alaris IV pump, Curlin pump, Joey enteral pump, Sapphire pump (varies by location)
• Medication dispensing: Pyxis system in inpatient facility; hospice home program medications generally administered by family/caregiver except IVs/injections managed by RN

Orientation

• Individualized based on discipline and needs
• Typically includes 1 day for equipment pick-up and online training, 2 days EPIC training, and 3-5 precepted shifts for visit nurses
• Orientation may require reporting to inpatient hospice facility
