Job Opportunity: Therapy/Rehabilitation - Medford, Oregon

Position Details

    • Patient load: Approximately 6 patients in an 8-hour day
  • Special Requirements: Inpatient Rehabilitation Facility (IRF) experience preferred

Experience Requirements

  • Years of Experience: Minimum 1 year in specialty
  • Prior Travel Experience: Not explicitly required; first-timers are welcome

Certifications Required (as applicable)

  • ACLS (Advanced Cardiac Life Support) - AHA or Canadian Heart & Stroke Foundation certification
  • BLS (Basic Life Support) - AHA or Canadian Heart & Stroke Foundation certification
  • PALS (Pediatric Advanced Life Support) - AHA or Canadian Heart & Stroke Foundation certification
  • NRP (Neonatal Resuscitation Procedure) - Current certification by AAP, AHA, or Canadian Paediatric Society
  • Chemotherapy certification (if applicable)
  • Electronic Fetal Monitoring (EFM) certification (if applicable)
  • State License (copy of professional license)
  • NURSYS Verification (for RNs)
  • IV Certification (LVN/LPN/Other, if applicable)
  • Management of Assaultive Behavior (MAB) (if applicable)
  • NIH Stroke Certification (every 2 years, if applicable)
  • CPI (Crisis Prevention Institute) certification (if applicable)

Competencies

  • Medication Test/Pharmacology Competency Exam (as applicable)
  • Competency Exams relevant to profession/specialty
  • Color Blind Testing (if required by unit)
  • Fetal Heart Rate Monitoring Assessment or EFM certification for Labor & Delivery RNs

Health Requirements

  • Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
  • Fit Mask testing (N95 or equivalent), updated annually (travel only)
  • Tetanus/Diphtheria/Tdap vaccine within 10 years or declination
  • Negative TB test or Chest X-Ray within 12 months, updated annually
  • MMR Vaccines or positive titers for Rubeola (Measles), Mumps, Rubella
  • Hepatitis B vaccination series or signed declination
  • Varicella vaccination or positive titer
  • 10 Panel Drug Screen within 90 days of start (no instant tests)
  • Seasonal Flu Vaccine or declination (required during flu season)
  • COVID vaccine or declination
  • Respiratory Safety Clearance (annual for travel candidates)

Background and Documentation Requirements

  • Background Check within 45 days of start, covering all locations lived/worked in past 7 years; updated annually
  • Education Verification required for all candidates
  • Employment Application covering at least last 5 years (7 years for APN/Subcontractors)
  • Minimum of 2 references covering 1 year of employment in specialty (6 months for CNAs)
  • Skills Checklists updated annually (if applicable)
  • State-specific searches and registry checks as required (e.g., Oregon Abuse Neglect Search, Washington State Patrol Search, OBRA Registry for CNAs in Washington)

Facility and System Requirements

  • Charting system: Epic
  • Scrub color: Any for RN/Allied; scrubs provided for OR, Critical Care, ED, PACU, Maternal Child
  • Parking: No fee
  • Floating Policy: Acknowledgement required; may float to other Providence facilities within reasonable distance
  • Badge photo required (passport style with neutral background)
  • Completion of Providence WorkRamp modules (Core, Specialty, Regional, and Annual Regulatory) as assigned based on profession and region
  • Epic training modules required for Oregon and Spokane candidates

This position is suitable for therapists with at least 1 year of experience in inpatient physical therapy, including those new to travel assignments.
