Job Description

• Work primarily in a 7-room inpatient/outpatient operating department with occasional float to a 26-room main OR and level one trauma unit.
• Experience with DaVinci XI surgical system is required.
• Must be skilled in scrubbing posterior eye surgeries and eye trauma cases.

• UHS Ohio Fingerprints - Background fingerprint check compliant with Ohio regulations
• Criminal Background Check - Within 6 months, including Social Security Trace, County searches, National Sex Offender check
• Drug Screen - 10-panel urine drug screen within 60 days prior to start (no instant tests)
• Competency Exam - Must score at least 80%, completed within 1 year prior to start
• Core Competencies - HIPAA, Joint Commission, OSHA topics; 80% passing score required, completed within 1 year prior to start
• UHS I-9 Attestation - Proof of eligibility to work in the U.S.
• Education Verification - Highest level of education verified through third party
• Employment Verification - Verification of employment within last 5 years through third party
• UHS Module Completion Certificate - Completion of required safety and compliance training modules prior to clearance
