Home Health/Hospice RN - Bristol, Connecticut

Job Details

• Specialty: Home Health/Hospice RN
• Location: Bristol, Connecticut
• Contract Length: 13 weeks
• Shift: Monday to Friday, 8:00 AM - 4:30 PM (40 hours guaranteed)
• Call Requirements: Every 4th weekend (flex a day from the M-F shift)
• Weekend/Holiday: Weekend call with callback within 20 minutes and visit within 30 minutes; holiday rotation with permanent staff
• Scheduling: Self-scheduling Monday to Friday, weekend rotation; possible full-time weekend position available

Experience & Certifications

• Required Specialty Experience: Minimum 1 year of home care or hospice experience
• Preferred Experience: Experience with Meditech home care EMR and Oasis highly preferred
• Prior Travel Experience: Not specified / not required
• Required Certifications:

  • BLS Certification
  • Valid Driver's License and Auto Insurance
  • Must be licensed and fingerprinted in Connecticut

Patient Population & Duties

• Patient Demographics: Adults and geriatrics only (no pediatrics)
• Common Diagnoses: Medication management, CFH, COPD, post-surgical, orthopedic total joints, CVA, dementia, hospice care
• Typical Caseload: Average 5-6 visits per day (weekly point system: start of care = 3 points, general visit = 1 point)
• Job Duties: Evaluations, treatment, plan of care development
• Respiratory Equipment: No ventilators; oxygen only

Additional Requirements

• Reliable car required
• Urban travel within a 20-minute radius
• Dress code: Any clean and professional scrub color
• Orientation: 1 week with preceptor
