Background & Compliance Requirements

• Criminal background check within 60 days of start, including:

• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
• Education verification required (degree or certification from accredited program)

Health & Immunization Requirements

• Physical exam within 1 year attesting ability to meet physical demands
• Tetanus/Diphtheria/Tdap vaccine or declination required
• Negative TB test or chest X-ray within required timeframes (Q-gold/T-spot or PPD)
• TB questionnaire required annually
• Titers and/or boosters required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Varicella
• Hepatitis B titer, vaccine series, or declination required
• Drug testing within 60 days of start (9-panel minimum, instant drug screens not accepted)
• Seasonal flu vaccination or approved exemption required for the 2025-26 season

Competencies & Testing

• Medication Test/Pharmacology Competency Exam (as applicable)
• Color Blind Testing (required)
• Abuse Reporting Competency (covering physical/sexual abuse, domestic violence, neglect/exploitation)
• Competency exams relevant to specialty (e.g., Sonography or Vascular Tech exams if applicable)

Orientation & Facility Specifics

• Orientation includes exams/tests such as Pharmacology Exam, Point of Care Testing, Airborne Precaution Mask Fitting (for RNs)
• Basic Knowledge Assessment Tool (BKAT) for RNs and LPNs in certain units
