Occupational Therapist (OT) - Travel Assignment

Location: Port Huron, Michigan
Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks

Job Details

• Full-time (1.0 FTE), Monday to Friday, 8:00 AM - 4:30 PM with alternating weekends
• Must be licensed to practice Occupational Therapy in Michigan
• Basic Life Support (BLS) certification required

Required Credentials & Compliance

• Licensure: Valid Michigan OT license
• Certifications: BLS
• Vaccinations & Health Requirements:

  • COVID-19: 2 vaccines plus booster or completed declination
  • MMR: Proof of 2 vaccines or positive titer
  • Hepatitis B: Required (declinations not accepted)
  • Influenza Vaccine: Required during flu season (Nov 1 - Mar 31) or declination (declinations not accepted at some locations)
  • Varicella: Vaccination or immunity titer required
  • Tdap: Required (declinations not accepted at some locations)
  • TB Test: Within 1 year of start date
  • Physical/Health Statement: Annual clearance to work required
• Drug Screening: Minimum 10-panel drug screen including THC within 60 days of start (special rules for CA locations)
• Fit Test: Annual respirator fit test recommended (may need to be completed prior to start)

Professional Requirements

• Driver’s License, State ID, or Passport required for submission
• National Criminal Background Check within 1 year of start date (some states require before assignment start)
• Prime Core Competencies Exam: Must score 80% or higher
• Prime Specialty Exam: Must score 80% or higher
• Prime Skills Checklist completion required
• Prime Agency Attestation and Form I-9 (employment eligibility verification) required
• Resume and 2 professional references (including 1 from a manager or supervisor) required at time of offer acceptance
• Name Verification Affidavit if there are discrepancies in credentials

Additional Notes

• Travelers receive a copy of the job description and acknowledgment form on their first day
• Badge photo taken on first day; no photo required prior
• Agency must upload required documents once credentialing is complete
