Job Opportunity: Occupational Therapist (Travel/Contract)

Job Description

• Start date: End of January or early February
• Primary work location: Montgomery Memorial Hospital (majority of time)
• Secondary location: Richmond Memorial Hospital (one 8-hour day per week)

Montgomery Memorial Hospital

• Coverage: Skilled Nursing Facility (SNF) / Acute Swing Bed with occasional outpatient
• Patient care: One-on-one
• Patient load: 6-10 patients per 8-hour shift
• Shift hours: 8:30 AM – 5:00 PM

Richmond Memorial Hospital

• Coverage: Outpatient/Acute split
• Patient focus: Orthopedic, gait/balance, post-surgical, aquatic therapy, neuro
• Patient care: One-on-one
• Patient load: 8-12 patients per 8-hour shift
• Shift hours: 8:00 AM – 4:30 PM

Experience Requirements

• Specialty: Occupational Therapy

Certifications & Licenses Required

• Valid State License (Primary Source Verification)
• NPI Number (must be obtained and verified prior to start)

Compliance & Documentation Requirements

• Government-issued photo ID (must be carried during hours of service)
• Background check within 1 year covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start and updated for renewals
• Physical exam (to meet agency standards, not facility required) within 1 year of start
• Immunizations and health screenings:

  • Tdap within 10 years
  • Negative TB test (PPD within 3 months or Qgold/TSPOT within 1 year) updated annually
  • Flu vaccine or approved medical/religious exemption during flu season (Nov 15 - Mar 31)
  • Measles, Mumps, Rubella, Varicella: Positive titer or 2 vaccines required
  • Hepatitis B series, titer, or declination
• Drug screen: 10-panel within 30 days of start
• Annual competencies and skills checklist as applicable
• Medication test/pharmacology competency exam as applicable
• Color blindness testing required for all candidates

Additional Notes

• Two clinical references required (one must be from a supervisor or higher)
• Complete employment application/resume required
• Facility-specific orientation modules must be completed prior to or shortly after start
• Candidate must be prepared for one-on-one patient care in both inpatient and outpatient settings
