Description

Job Opportunity: RN Medical/Surgical

Location: Mason City, Iowa
Employment Type: Long-Term Assignment (LTA)
Shift: 7:00 PM – 7:30 AM, every third weekend
Contract Length: 13 weeks


Required Experience

  • Specialty: RN Medical/Surgical

Certifications & Licenses (Required for Onboarding)

  • BLS (American Heart Association certification)

Health & Immunization Requirements

  • Fit Test
  • Trinity 10 Panel Drug Screen (completed within 30 days of start; no THC)
  • COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption except for CA or IL)
  • MMR (proof of 2 vaccinations or IgG titer)
  • Hepatitis B vaccination series (at least first vaccination prior to start)
  • Influenza Vaccine (proof or declination for 2024-2025 season)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
  • Varicella (proof of vaccination or IgG titer)
  • Physical/Health Statement (exam within last year confirming fitness to work)
  • Tdap vaccine (current, non-expired; declination accepted if no pediatric interaction)

Professional & Compliance Requirements

  • Trinity Code of Conduct Form (signed)
  • Trinity I9 form (completed by agency)
  • Confidentiality and System Security Agreement (signed)
  • Integrity and Audit Services Form (signed shortly before start)
  • Non-Employee Upload Form (pages 1-3 and 28 completed)
  • Conflict of Interest Disclosure Form (signed)
  • Criminal Background Check (federal, nationwide, county for last 7 years; dated within 30 days of start)
  • Education Verification (highest academic degree or diploma verified)
  • EPLS (Excluded Parties List System) check dated within 30 days
  • GSA/OIG (Office of Inspector General) exclusion check dated within 30 days
  • Joint Commission CORE Exam (within last year)
  • NSO (National Sex Offender) check dated within 30 days (includes aliases and locations)
  • System Access Form (completed and uploaded one week before start)
  • Skills Checklist (provided by agency for clinical contractors)
  • Child and Adult Dependent Abuse Training (Iowa only)
  • Iowa Sing Report (Iowa-specific criminal history, sex offender, dependent adult & child abuse registry check within 30 days prior to start)
  • OFAC (Office of Foreign Assets Control) sanctions check within 30 days of start

This position requires thorough credentialing and compliance documentation prior to onboarding. The assignment is a night shift with every third weekend coverage, suitable for experienced Medical/Surgical RNs seeking a 13-week contract in Iowa.
