Job Description

• Work primarily in the outpatient Phase 2 Cardiac Rehab program.
• Phase 2 involves cardiac patient therapy where patients with cardiac diagnoses:

  • Exercise in a gym setting
  • Receive education
  • Have EKG and vital signs monitored
  • Care plans developed
  • Wellness goals set
• Phase 2 outpatients attend 3 times per week (Monday, Wednesday, Friday) for 12 weeks per CMS rules.
• The outpatient gym requires a minimum of 2 qualified staff members on full gym days, with at least one being an RN.
• Typical shifts for agency RNs are four 10-hour shifts per week, 6:30 AM to 5:00 PM, with some flexibility for 8-hour shifts.
• The gym operates from 6:30 AM to 3:00 PM on full gym days; last patient session ends at 1:00 PM.
• Additional non-direct patient care tasks include charting care plans, collaborating with medical directors, scheduling patient appointments, and follow-up calls.

Program Details
