Job Opportunity: Surgical Technician (CST- OB)

Location: Mishawaka, Indiana
Employment Type: Long-Term Assignment (LTA)

Required Credentials & Certifications

• Certified Surgical Technologist (CST) – Must be current and valid
• BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel
• License/Certificate Verification – Must be dated within 30 days of start
• Trinity Specialty Specific Competency Testing – Specialty exam completed within the last year
• Trinity Skills Checklist – Provided by agency for clinical contractors

Health & Immunization Requirements

• Fit Test
• Trinity 10 Panel Drug Screen – Completed within 30 days of start, must include specific substances; no THC allowed
• COVID-19 Vaccine – Proof of booster by Sept 2025, or declination/exemption (declination not accepted in CA or IL)
• Influenza Vaccine – Proof or declination/exemption for current flu season (declination not accepted in CA or IL)
• MMR – Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B – Must have started vaccination series prior to start; titers not accepted
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – TB testing with chest x-ray if positive (within last year for Indiana)
• Varicella – Proof of vaccination or IgG titer lab report
• Physical/Health Statement – Documentation from exam within last year confirming fitness to work
• Tdap – Current, non-expired vaccine; declinations accepted if no pediatric interaction

Professional & Compliance Requirements

• Trinity Code of Conduct Form – Signed form required
• Trinity I9 – Standard employment eligibility verification
• Confidentiality & Security Agreement – Signed agreement regarding patient and proprietary information
• Integrity and Audit Services Form – Signed shortly before start
• Conflict of Interest Disclosure Form – Signed form required
• Criminal Background Check – Federal, nationwide, county checks for last 7 years; SSN verification; dated within 30 days of start
• Education Verification – Highest academic degree or diploma verified by third party screening
• EPLS (Excluded Parties List System) Check – Dated within 30 days of start
• GSA/OIG Exclusion Check – Dated within 30 days of start
• Joint Commission CORE Exam – Completed within last year
• NSO (National Sex Offender) Check – Dated within 30 days of start, including aliases and locations
• System Access Form – Must be completed and uploaded one week before start
• OFAC (Office of Foreign Assets Control) Sanctions Check – Completed within 30 days of start
• Orientation Guide – Must be completed
