Job Description

• Specialty: Physical Therapist (PT) Home Health
• Coverage Area: 4 counties (Cherokee, Spartanburg, Union, and a small part of Greenville)
• Shift: Days, 4x10-hour shifts (Wednesday, Thursday, Friday, Saturday)
• Minimum Units: 7.5 units per shift
• Start Date: ASAP
• On-call: Not required
• Floating: Yes, within scope to meet facility needs (mileage paid)
• Block Schedule: Not accommodated
• Local Candidates: Not accepted (75 miles radius rule)
• Pending State License: Accepted but must be obtained prior to starting
• Attendance Policy: More than two missed shifts per 13-week contract without reasonable explanation may lead to contract cancellation
• Special Requirement: Must have PT Home Health experience

• Fit Mask (N95) Fit Testing: Within 1 year of start, updated annually
• Immunizations and Health Documents:

  • Negative TB test or chest X-ray within 3 months of start
  • Measles (Rubeola), Mumps, Rubella: 2 vaccines, positive titer, or approved exemption
  • Hepatitis B: Vaccine series, positive titer, or declination
  • Varicella: 2 vaccines, positive titer, or approved exemption
  • Flu Vaccine or Declination (seasonal requirement)
  • COVID-19: Dose 1 and 2 required or declination form
