Job Opportunity: RN - Home Health

Location: Tuscaloosa, Alabama
Employment Type: Home Health (Clinical, Patient-facing)
Contract Length: 13 weeks

Job Description

• Work hours: 8 am – 5 pm
• No weekend requirement; holidays as required
• Patient volume: 5-7 patients per day
• Must provide proof of auto insurance at submission
• Collaborate with Home Health RN Care Manager and interdisciplinary team
• Provide skilled nursing care, education, treatment, and emotional support to patients
• Monitor and assess patient care to ensure highest level of functioning
• Communicate changes in patient condition and progress to care team and physicians
• Participate in plan of care development, implementation, and modification
• Perform skilled assessments and document findings accurately and timely (OASIS)
• Measure wounds weekly and maintain accurate medication lists
• Participate in performance improvement plans and interdisciplinary team (IDT) meetings
• Participate in on-call rotation
• Must be willing to float to other units/hospitals as needed

Requirements

Education

• Associate Degree in Nursing

Licenses & Certifications

• Registered Nurse license in Alabama or Compact license (No temporary licenses accepted)
• AHA BLS or Red Cross certification
• NURSYS registration

Experience

• Minimum 2 years nursing experience preferred
• Will consider 18 months of experience under highest professional license
• Must have at least 1 year of experience in the specialty (Home Health Nursing)
• Prior travel experience: Not explicitly required

Skills & Knowledge

• Experience with Homecare Homebase (HCHB) EMR required
• Ability to educate patients and families on home health care
• Strong communication skills with physicians and care team
• Ability to assess patient outcomes and suggest improvements

• Guaranteed hours: 36 per week
