Occupational Therapist (OT) - Home Health (Days)

Job Overview

As an Occupational Therapist on the Home Health team, you will provide clinical expertise and compassionate care directly to patients in their homes. You will develop and deliver personalized therapy plans to help patients regain independence, improve quality of life, and achieve their goals. Collaboration with physicians and the care team is essential to provide hands-on treatment, education, and support based on best clinical practices.

Responsibilities

• Conduct home visits for patients requiring occupational therapy services.
• Perform ongoing interdisciplinary assessments of patients.
• Evaluate and assess patient functioning using appropriate tests.
• Develop occupational therapy procedures and functional activities aligned with physician treatment plans and patient evaluations.
• Create and administer treatment plans, including environmental modifications.
• Oversee the use of treatment modalities, prosthetic devices, and therapeutic appliances.

Qualifications

Education

• Graduate of an occupational therapy education program accredited by ACOTE (Accreditation Council for Occupational Therapy Education) or its successor organizations.

Certification & Licensure

• Eligible to take or have completed the NBCOT (National Board for Certification in Occupational Therapy) entry-level certification exam (highly preferred).
• Licensed or regulated as an Occupational Therapist in the practicing state (if applicable).
• Valid driver’s license with automobile insurance as required by state or organization.
• Maintain valid Basic Life Support (BLS) Healthcare Provider certification.
• Mandatory reporter for Child & Dependent Adult Abuse.

Experience

• Minimum of 1 year experience caring for a similar patient population.

Work Schedule

• Monday to Friday, 8:00 AM – 4:30 PM
