Job Details

• Shift: 7 PM to 7 AM
• Schedule: Every other weekend
• Hours: 36 hours per week
• Specialty Experience Required: NICU/PICU experience
• Certifications Required:

  • BLS (Basic Life Support)
  • ACLS (Advanced Cardiovascular Life Support)
  • NRP (Neonatal Resuscitation Program)
  • PALS (Pediatric Advanced Life Support)
  • RRT (Registered Respiratory Therapist credential)

Required Credentials & Compliance

• Current and valid professional license verification
• Fit Test
• Trinity 10 Panel Drug Screen (includes multiple substances, no THC)
• COVID-19 Vaccine (booster required, no declination accepted in IL)
• MMR vaccination or immunity proof
• Hepatitis B vaccination series (titers not accepted)
• Influenza vaccine (seasonal, no declination accepted in IL)
• Varicella vaccination or immunity proof
• 2 Step PPD OR IGRA (Quantiferon/Tspot) for TB screening
• Physical/Health Statement confirming fitness to work (within last year)

• Confidentiality and Information Security Agreement
• Integrity and Audit Services Form
• Conflict of Interest Disclosure Form
• Criminal Background Check (last 7 years, including SSN verification)
• Education Verification (highest level of academic education)
• EPLS (Excluded Parties List System) check
• GSA/OIG Exclusion check
• Joint Commission Testing (CORE Exam within last year)
• NSO (National Sex Offender) check
• System Access Form (to be completed one week before start)
• Skills Checklist (clinical contractors only)

Experience Requirements
