Job Opportunity: Registered Nurse (Travel/Contract)

Job Description

• Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits with periodic office meetings.
• Average patient load: 5-8 patients per day.
• Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
• Schedule: Monday to Friday, 8 AM to 5 PM.
• Weekend rotation: None.
• On-call: None.
• Travel: Must be willing to travel up to 75 miles from home clinic, averaging about 200 miles per week.
• EMR system: HomeCare HomeBase (HCHB) and OASIS proficiency required.

Contract Details

• Length: 13 weeks (approximately 13 weeks).
• Employment Type: Travel/Contract.
• Local candidates accepted at the same rate.
• No pending licenses accepted; must have license in hand.

Experience & Requirements

• Minimum 1 full year of recent Adult Home Health or Hospice experience (whichever is applicable).
• First-time travelers not accepted; must have recent specialty experience.
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Proficiency with HCHB within the last 6 months required; retraining may be required if last used over 6 months ago.
• RNs must be proficient in OASIS.
• Reliable transportation and current driver's license required.
• Agency-required liability insurance required.

Certifications & Licenses

• CPR (BLS) current certification required.
• Must have a valid nursing license for Pennsylvania.

Compliance & Documentation Requirements

• Background checks including 7-year history, SS trace, county, nationwide, sex offender, MVR, and state-specific checks.
• Fingerprinting required per state regulations (PA-specific fingerprinting required).
• Drug testing: 10-panel drug screen required.
• Health documents: Physical exam within 12 months, negative TB or chest X-ray within 12 months, immunizations/titers for Rubeola, Mumps, Rubella, Hepatitis B, Varicella, and COVID vaccination or declination.
• References: Two references required.
• Work history verification: At least two most recent employers or last 7 years.
• Various facility-specific compliance documents and training must be reviewed and signed prior to start (e.g., HIPAA, Code of Conduct, Compliance Training, Privacy Policy, Home Health Scenarios, etc.).

Additional Notes

• Dress code: Scrubs.
• Productivity expectations: 30 points per week (based on patient visits and documentation).
• This is a critical backfill position; candidates must have home health and HCHB experience.
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not billable).

This position is ideal for experienced Registered Nurses with recent home health or hospice experience who are comfortable with travel within a 75-mile radius and proficient in required EMR systems.
