Job Opportunity: Registered Nurse - Home Health
Location: Indianapolis, Indiana
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits with periodic office meetings.
- Average patient load: 5-8 patients per day.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Must be willing to travel up to 75 miles from the home clinic, with weekly travel estimated between 100-200 miles.
- Charting system: Proficiency required in HomeCare HomeBase (HCHB) and OASIS 3 (RNs, PTs, OTs). Retraining required if not used within the last 6 months.
- Schedule: Monday to Friday, 8 AM to 5 PM.
- Weekend/on-call rotation every 3-4 weeks.
- Dress code: Scrubs.
Requirements
- Specialty: Registered Nurse with Home Health experience.
- Experience: Minimum 1 full year of Adult Home Health experience or 1 full year Hospice experience (whichever is higher).
- First-timers: Not accepted; must have recent specialty experience.
- Certifications: Current CPR and BLS required.
- Licensure: Must have license in hand; pending licenses not accepted.
- Transportation: Reliable transportation, current driver's license, and agency-required liability insurance required.
- Prior Travel Experience: Not explicitly required.
Productivity Expectations
- Weekly productivity units: 30 points.
- Patient visits per day: 5-10 patients, equating to roughly 6-8 points daily.
Additional Information
- Local candidates accepted at the same rate.
- Mileage reimbursed at IRS standard rate; compensation is from patient home to patient home (commute to/from first and last patient is not billable).
- Some positions may have a Baylor Shift schedule (4x10s, Friday-Monday), but most are standard 5x8, Monday-Friday.
Compliance and Documentation Requirements
- Must complete a variety of background checks including:
- 7-year standard background check with SS trace, county, nationwide, sex offender, and state-specific searches.
- EPLS/GSA/SAM and HHS/OIG screenings.
- Fingerprinting as required by state (varies by location).
- Education verification required.
- Drug testing: 10-panel drug screen required.
- Health documents required:
- Physical exam within 12 months.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- Flu vaccination or declination for current season.
- COVID vaccination series or declination.
- References: Two references required for all placements.
- Work history verification: Must verify at least two most recent employers or last 7 years if only one employer.
- Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start.
- Must be proficient or complete training in OASIS E course if not used within last 6 months.
